Basilar thumb osteoarthritis at the trapeziometacarpal joint is the second most common location of osteoarthritis in the hand. Arthritis and instability of the thumb MCP joint can be quite debilitating, given the crucial role the joint plays in thumb dexterity. Advanced basilar thumb osteoarthritis often leads to collapse of the first ray with compensatory hyperextension of the thumb metacarpophalangeal (MCP) joint and is accompanied by arthritis in the scaphotrapezialtrapezoid joint. Here the authors discuss the management of MCP hyperextension and scaphotrapezoidal osteoarthritis in the setting of basilar thumb osteoarthritis.

The present study aims to determine if morphological differences of the scaphoid, trapezoid, and second metacarpal are associated with thumb osteoarthritis by comparing three-dimensional bone models of healthy controls and osteoarthritis patients. Fifty-eight patients with moderate to severe thumb osteoarthritis (modified Eaton ≥ II) and 35 healthy controls from a larger completed investigation were examined. To quantify morphological variations, a statistical shape model was created that classified binary with respect to the Eaton grading system: non-osteoarthritis or moderate/severe osteoarthritis (II-IV).

This retrospective study compares the outcomes of trapeziectomy and Weilby suspensionplasty procedure versus implant arthroplasty using the TOUCH prosthesis for basilar thumb arthritis in an Asian population. A total of 15 consecutive thumbs in 13 patients were included in this study. Six patients (2 male, 4 female, mean age of 62 years old) underwent trapeziectomy and Weilby suspensionplasty procedure.

Two patients with thumb carpometacarpal instability were stabilized using a suture suspension device. Both patients had symptomatic thumb carpometacarpal instability in the setting of clinical hyperlaxity without known connective tissue disorder that was recalcitrant to nonsurgical modalities. Both patients had significant, lasting improvement in their pain and function with excellent radiographic outcomes.
