Published: April 2022

Objective: We aimed to develop an effective nomogram model for predicting the risk of preeclampsia in twin pregnancies.

Methods: The study was a retrospective cohort study of women pregnant with twins who attended antenatal care and labored between January 2015 and December 2020 at the Fujian Maternity and Child Health Hospital, China. We extracted maternal demographic data and clinical characteristics. Then we performed the least absolute shrinkage and selection operator regression combined with clinical significance to screen variables. Thereafter, multivariate logistic regression was used to construct a nomogram that predicted the risk of preeclampsia in twin pregnancies. Finally, the nomogram was validated using C-statistics (C-index) and calibration curves.

Results: A total of 2,469 women with twin pregnancies were included, of whom 325 (13.16%) had preeclampsia. Multivariate logistic regression models revealed that serum creatinine, uric acid, mean platelet volume, high-density lipoprotein, lactate dehydrogenase, fibrinogen, primiparity, pre-pregnancy body mass index, and regular prenatal were independently associated with preeclampsia in twin pregnancies. The constructed predictive model exhibited a good discrimination and predictive ability for preeclampsia in twin pregnancies (concordance index 0.821).

Conclusion: The model for the prediction of preeclampsia in twin pregnancies has high accuracy and specificity. It can be used to assess the risk of preeclampsia in twin pregnancies.

Twin pregnancies consisting of one normal fetus and one complete mole are very rare. The main concerning risks associated with the continuation of such pregnancy are hyperthyroidism, theca lutein cysts, preeclampsia, and the development of GTD (gestational trophoblastic disease) spectrum (neoplasia) in the mother, which is due to high human chorionic gonadotropin (HCG) values, and intrauterine death and prematurity in the coexistent normal fetus. We report the successful outcome of conservative management in a healthy mother and baby.

Peripartum cardiomyopathy (PPCM) is a rare, but serious condition, with a non-negligible risk of adverse events. Several risk factors for PPCM have been individuated over the years, including Afro-American ethnicity, preeclampsia, advanced maternal age, genetic predisposition, multiparity, twin pregnancy, obesity, smoking and diabetes. However, PPCM pathophysiology is still poorly understood, thus making it challenging to develop disease specific therapies.

Objectives: Placental growth factor (PlGF) has been reported as a good biomaker for the prediction of preeclampsia occurring in the short term in singleton pregnancies, in women presenting with clinical suspicion of preeclampsia. This study aims to evaluate the predictive value of the PlGF in twin pregnancies.

Methods: Twin pregnancies with clinically suspected preeclampsia (24 weeks 0 days-36 weeks 6 days of gestation) were enrolled in this study.

Maternal smoking, including both traditional cigarettes and electronic ones, is a significant modifiable risk factor associated with adverse perinatal outcomes, especially in twin pregnancies. This narrative review aims to explore the impact of maternal smoking on obstetric and neonatal outcomes in twin pregnancies, which inherently carry a higher risk of complications. A literature search was conducted using the PubMed and EMBASE databases, selecting studies published between January 1994 and October 2024.

Impact of stage 1 hypertension before 20 gestational weeks on pregnancy outcomes in twin pregnancies: a retrospective cohort study.

Division of Fetal Medicine, Prenatal Diagnosis Department, International Peace Maternity & Child Health Hospital, Shanghai Jiao Tong University School of Medicine, Shanghai, 200030, China.

To explore the association between stage 1 hypertension before 20 weeks of gestation and twin pregnancy outcomes and to examine whether the association varies by prepregnancy BMI, conception mode, or chorionicity, we conducted a single-center, retrospective, hospital-based cohort study. All women who delivered twins between July 2014 and December 2020 were recruited. Logistic regression and linear regression models were used to identify associations.
