Background: Ex situ machine perfusion of the donor liver, such as dual hypothermic oxygenated machine perfusion (DHOPE), is increasingly used in liver transplantation. Donation after circulatory death is helping to expand the donor pool for heart transplantation. Nevertheless, these hearts are more susceptible to myocardial edema and decision of accepting the organ can be a challenge for the heart transplant team. Hemodynamic and echocardiographic criteria are used routinely, but there is still a lack of strong evidence that supports the decision-making in particular situations.

We report the case of a young patient with severe hypoxic brain injury after cardiopulmonary resuscitation, resulting in brain death/death by neurologic criteria (BD/DNC). Consistent with the patient's expressed wishes, treatment was sustained to facilitate organ donation. However, in the context of a severe post-resuscitation syndrome and physiological disturbances resulting from BD/DNC, refractory circulatory shock ensued.

Despite that, the number of organs transplanted from these donors remains low due to concerns about their function and a lack of an objective assessment at the time of donation.
