Unlabelled: Kleine-Levin syndrome is a rare neurologic disorder of unknown etiopathogenesis, characterized by abrupt onset and remission of attacks of hypersomnia and cognitive dysfunctions. Psychiatric symptoms are frequently present, ranging from disinhibited sexual behavior and eating disorders to hallucinations, anxiety, mood alterations, and derealization. A vast range of attack-related dysautonomic signs and symptoms are reported but remain poorly described. We describe a patient with Kleine-Levin syndrome with sleep attacks dominated by marked dysautonomic features. We briefly review similar clinical cases and suggest that the hypothalamus may play a central role in the genesis of autonomic dysfunction in Kleine-Levin syndrome.

Article Synopsis
  • * The review included 20 studies with a total of 2,232 participants, showing common patterns of hypoperfusion in certain brain regions during rest and hyperperfusion during cognitive tasks in those with ADHD.
  • * Key findings indicate that while certain areas like the prefrontal cortex often experience reduced blood flow at rest, other regions may show increased blood flow during tasks, suggesting the need for further research to understand these variations better.

Article Synopsis
  • - KCNMA1-linked channelopathy leads to neurodevelopmental disorders, epilepsy, and non-epileptic episodes characterized by specific facial, behavioral, and physical symptoms.
  • - A review of 14 videos highlighted typical episode features: facial changes, behavioral arrest, loss of postural control, and quick recovery without drowsiness, with episodes often triggered by emotions.
  • - Distinguishing KCNMA1-related attacks from other conditions like paroxysmal dyskinesia and cataplexy will improve accurate diagnosis and targeted treatment for affected individuals.

Connecting clinicians and patients: The language of narcolepsy.

Sleep Med

Center for Sleep Medicine, Pulmonary Medicine, Mayo Clinic Arizona, Scottsdale, AZ, USA.

Narcolepsy is a rare lifelong sleep disorder characterized by excessive daytime sleepiness with variable expression of cataplexy, sleep paralysis, sleep-related hallucinations and disrupted nocturnal sleep. Affected individuals also experience additional impairing symptoms, including (but not limited to) difficulties with attention, memory and concentration, brain fog, mood instability and fatigue, with a substantial impact on everyday life. Diagnostic delays of up to 10 years are common, primarily due to the substantial heterogeneity in clinical presentation of narcolepsy symptoms and presence of significant comorbidities.
