Orthoses for neurological ankles.

Pract Neurol

Rehab Medicine Department, Cambridge University Hospitals NHS Foundation Trust, Cambridge, UK

Published: August 2022

Patients with weakness or abnormal posture of their lower leg may benefit greatly from appropriate orthoses. This paper describes the sorts of problems that can be helped in neurological practice and the range of devices commonly used, and also highlights some of the factors influencing selection. With greater understanding of their use, clinicians will feel more confident about referring patients for early orthotic assessment.

Article Synopsis
  • A dynamic elastomeric fabric orthosis shows potential as a non-drug treatment for improving motor symptoms in Parkinson's disease patients, specifically aimed at enhancing manual dexterity.
  • In a study involving 60 participants, the experimental group used the orthosis for two months, with assessments measuring manual dexterity before and during its use.
  • While some participants exhibited short-term improvements in manual dexterity while wearing the orthosis, these enhancements were not maintained once the device was removed.

Background: Ankle-foot orthoses are commonly prescribed to address motor impairments in individuals with neurological disorders. Proper alignment of Ankle-foot orthoses, commonly assessed by the ground reaction force in relation to the knee joint center and the shank-to-vertical angle, is crucial for their effectiveness but is time-consuming. This study validates automated pose estimator DeepLabCut on measuring these metrics using 2D videos with force vector overlay in individuals with neurological disorders wearing ankle-foot orthoses.

Dystonia is a neurological motor disorder characterised by involuntary and uncontrollable muscle contractions, tension, twisting, and tremors. The aim of the present study was to analyse the improvement in dystonic contractions in patients with dystonia following the use of a Hegab temporomandibular joint splint (HTS). The Fahn-Marsden Dystonia Movement Scale (DMS) and Disability Scale were used in the current study to evaluate dystonia.

Objective: Provide data on a cross-sectional study of neurodevelopmental delays in positional plagiocephaly (PP) based on severity and flattening side on early childhood patients.

Methods: Neurodevelopment of 408 PP in patients with an age range of 3 to 59 months was assessed with Battelle Developmental Inventory (BDI) tests during and after their cranial orthotic treatment. Data obtained were compared with the cranial anthropometric measurements taken the same day the test was made, and results were segregated by flattening side, grouped by diagnosis and severity, segmented by age group, and split by sex.
