Acute Coronary Syndrome with ST segment elevation requires rapid reperfusion, which is why the timely identification of these conditions, based on the patient's symptoms and the correct interpretation of the EKG, is essential for therapeutic decisions. We report three patients with True Posterior Infarction and their difficult EKG diagnosis. The classic 12-lead EKG method is often not capable of supporting the diagnosis, so we recommend the use of posterior leads and mirror images observing inverted V1-V2-V3 leads on the classic EKG.

Alexander's law states that spontaneous nystagmus increases when looking in the direction of fast-phase and decreases during gaze in slow-phase direction. Disobedience to Alexander's law is occasionally observed in central nystagmus, but the underlying neural circuit mechanisms are poorly understood. In a retrospective analysis of 2,652 patients with posterior circulations stroke, we found a violation of Alexander's law in one or both directions of lateral gaze in 17 patients with lesions of unilateral lateral medulla affecting the vestibular nucleus.

Objective: To evaluate the efficacy of butylphthalein injection combined with alteplase thrombolysis in patients with acute cerebral infarction (ACI) and its effects on lipoprotein-associated phospholipase A2 (Lp-PLA2) and CXC chemokine ligand 16 (CXCL16) levels.

Methods: A total of 127 ACI patients admitted to Shandong Provincial Third Hospital between March 2020 and June 2023 were included and divided into a butylphthalein group (n = 67) and a control group (n = 60) based on their treatment regimen. All patients received basic treatment.
