Schwannomas are classified as neurogenic tumors and are the most frequent nerve sheath tumors in the paravertebral mediastinum. Recently, the addition of endobronchial ultrasound-guided intranodal forceps biopsy (EBUS-IFB) using standard-sized biopsy forceps (SBFs) to endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) for metastatic lymph nodes in lung cancer patients reportedly improved the quality and quantity of the obtained specimens without significant complications. However, reports on the usefulness of this technique for benign diseases remain scarce.

Robotic-assisted thoracic surgery: left upper lobe sleeve lobectomy for an endobronchial tumour.

Multimed Man Cardiothorac Surg

February 2024

Barts Thorax Centre, St. Bartholomew's Hospital, West Smithfield, EC1A 7BE, London, UK.

Pulmonary sleeve resection is a technically challenging procedure entailing expertise to perform via a minimally invasive approach. Robotic thoracic surgery with its three-dimensional high-definition imaging and true depth perception enables clarity of anatomical structures and, in conjunction with its articulated instruments, provides greater dexterity compared with video-assisted thoracoscopic surgery, allowing complex manoeuvres in limited spaces. We present the case of a 22-year-old woman who presented with recurrent chest infections, dyspnoea on exertion, cough, loss of appetite and weight loss.

Article Synopsis
  • Endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) is effective for diagnosing advanced-stage lung cancer, but sometimes produces samples with blood contamination, making them inadequate for treatment decisions.
  • Researchers introduced a larger 1.9-mm standard-sized forceps biopsy (SFB) to see if it could enhance specimen quality compared to the smaller 0.96-mm miniforceps biopsy (MFB).
  • In a study with 60 patients, SFB demonstrated higher diagnostic yield and significantly larger sample size compared to MFB, showing its potential for better quality specimens necessary for effective treatment planning.

Comparison of cryobiopsy and forceps biopsy for the diagnosis of mediastinal lesions: A randomised clinical trial.

Pulmonology

September 2024

Department of Respiratory Disease, Xinqiao Hospital, Third Military Medical University, Chongqing, China. Electronic address:

Introduction: Endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) is the standard approach for lung cancer staging. However, its diagnostic utility for other mediastinal diseases might be hampered by the limited tissue retrieved. Recent evidence suggests the novel sampling strategies of forceps biopsy and cryobiopsy as auxiliary techniques to EBUS-TBNA, considering their capacity for larger diagnostic samples.
