Background: Little is known about pain trajectories in the emergency department (ED), which could inform the heterogeneous response to pain treatment. We aimed to identify clinically relevant subphenotypes of pain resolution in the ED and their relationships with clinical outcomes.

Methods: This retrospective cohort study used electronic clinical warehouse data from a tertiary medical center. We retrieved data from 733,398 ED visits over a 7-year period. We selected one ED visit per person and retrieved data including patient demographics, triage data, repeated pain scores evaluated on a numeric rating scale, pain characteristics, laboratory markers, and patient disposition. The primary outcome measures were hospitalization and ED revisit.

Results: 28,105 adult ED patients were included with a total of 154,405 pain measurements. Three distinct pain trajectory groups were identified: no pain (57.1%); moderate-to-severe pain, fast resolvers (17.9%); and moderate pain, slow resolvers (24.9%). The fast resolvers responded well to treatment and were independently associated with a lower risk of hospitalization (adjusted odds ratio [aOR], 0.75; 95% confidence interval [CI], 0.70-0.81). By contrast, the slow resolvers had lingering pain in the ED and were independently associated with a higher risk of ED revisit (aOR, 2.65; 95%CI, 1.85-3.69). This group also had higher levels of inflammatory markers, including a higher leukocyte count and a higher level of C-reactive protein.

Conclusions: We identified three novel pain subphenotypes with distinct patterns in clinical characteristics and patient outcomes. A better understanding of the pain trajectories may help with the personalized approach to pain management in the ED.

Background: To investigate the effect of Midnight-noon Ebb-flow combined with five-element music therapy in the continuous nursing of patients with chronic wounds.

Methods: From March 2022 to November 2023, we recruited 50 eligible chronic wound patients and randomly divided them into two groups according to a random number table: the experimental group (n = 25) and the control group (n = 25). The control group was treated with conventional nursing measures.

Variations in the development of carpal bones are uncommon, with the scaphoid bone typically forming from the fusion of the os centrale carpi and the radial chondrification center during embryogenesis. A bipartite scaphoid is a rare congenital disorder that occurs when these ossification centers fail to fuse, with a prevalence ranging from 0.1% to 0.

Background: Smartphone overuse is associated with both psychological and physical health problems, including depression and musculoskeletal disorders. However, the association between smartphone overuse and neck pain remains unclear. We performed a meta-analysis to examine the relation between smartphone overuse and neck pain, and to identify high-risk usage patterns.

Avascular necrosis (AVN) of the hip, a rare cause of pelvic pain in the third trimester of pregnancy, often presents with nonspecific symptoms that resemble common musculoskeletal conditions. This ambiguity, coupled with concerns about the safety of magnetic resonance imaging (MRI) during pregnancy, can hinder timely diagnosis. We report a unique case of a 32-year-old primigravida diagnosed with a hip fracture and bilateral AVN of the femoral head in the immediate postpartum period, a complication seemingly not previously documented in pregnant patients.

Heterotopic pregnancy is defined as the concurrent presence of both an intrauterine pregnancy and an extrauterine (typically ectopic) pregnancy. This report presents the case of a 36-year-old female patient who presented to the emergency department with lower abdominal pain. A comprehensive evaluation, including transabdominal and transvaginal ultrasound imaging, revealed a heterotopic pregnancy at an estimated gestational age of six weeks and two days.
