Published: December 2022

This study describes a large nosocomial outbreak of infections (CDI) dominated by ribotype (RT) 046 in a Swedish hospital. The present study aimed to examine the pathogenicity of this RT, explore epidemiological links by whole genome sequencing (WGS), and evaluate different interventions implemented to stop the outbreak. Clinical isolates ( =  366) collected during and after the outbreak were ribotyped and 246 isolates were subjected to WGS. Medical records of patients infected with the seven most common RTs were evaluated. RT046 was spread effectively throughout the hospital and was the most common among the 44 different RTs found (114/366 isolates). Infection with RT046 was associated with higher mortality compared to other strains (20.2% to 7.8%), although there were no differences in concomitant disease, age or antibiotic treatment. To control the outbreak, several measures were successfully implemented.

Article Synopsis
  • - The study investigated the prevalence and characteristics of C. difficile infections in children and adolescents with diarrhea in northeastern Brazil, revealing a significant infection rate of 30.4% among samples tested.
  • - A total of four toxigenic strains were identified, with most samples showing susceptibility to metronidazole and vancomycin, while diarrhea associated with these infections lasted an average of 11 days and was often severe.
  • - The research indicates that comorbid chronic diseases were key risk factors for these infections, underscoring the need for further awareness and understanding of C. difficile's impact on pediatric health in the region.

Article Synopsis
  • Nosocomial diarrhoea linked to antibiotic use and weakened immune systems is rising globally, particularly in children, but research on this issue is scarce.
  • This report details the clinical symptoms and genetic analysis of Clostridioides difficile infection (CDI) in a young girl who underwent a liver transplant at 7 months old.
  • This study identifies the 046 ribotype strain as a novel cause of diarrhoea in pediatric patients, marking the first documentation of its occurrence in this age group.

Objective: The motility and genotype of the flagellin fliC and fliD genes were investigated in 82 Clostridioides difficile isolates belonging to the ribotypes (RTs): 027 (n = 41), 176 (n = 17), 023 (n = 8), 017 (n = 6) and 046 (n = 10). The reference C. difficile strains 630 and M120 were included as controls for the motility assay.
