Unlike classical antidepressants, psychedelics such as psilocybin have been shown to induce a rapid antidepressant response. In the wake of this development, interest has emerged in ultra-fast, short-acting psychedelics such as 5-methoxy-N,N-dimethyltryptamine (5-MeO-DMT) and N,N-dimethyltryptamine (DMT) with the expectation that these can produce rapid antidepressant effects following an intense but brief psychedelic intervention. The current paper reviews the clinical pharmacology of 5-MeO-DMT and DMT and their potential benefits and challenges in the treatment of depression.

Article Synopsis
  • Arketamine (R-ketamine) has shown potential for prolonged antidepressant effects and better safety compared to esketamine (S-ketamine), though it has been less studied.
  • A review of 20 studies with 410 subjects indicated arketamine's effectiveness for pain management and depression, but many studies were small and informal.
  • While arketamine generally has a favorable safety profile, more substantial research is needed to better understand its therapeutic potential and proper clinical usage.

Maternal antidepressant use has increased during the past 2 decades, with venlafaxine emerging as a common agent during pregnancy. Both venlafaxine and its active metabolite possess prolonged half-lives in adults; however, abrupt discontinuation may lead to withdrawal including irritability, jitteriness, lethargy, restlessness, and insomnia. The drug and its metabolite readily cross the placenta, posing additional considerations during pregnancy.
