Although the incidence of rheumatic fever has declined significantly over the last decade, testing for antibodies to streptococcal extracellular enzymes maintains an important role in differentiating this disease from others with similar characteristics. Detection of antibodies to streptolysin O and DNase-B remain the more popular single antibody tests while the streptozyme test, which detects antibodies to five distinct streptococcal extracellular products, has been increasingly used in recent years as a screening test. Several new procedures detecting antibodies to different somatic antigens have been developed, the most promising of which seem to be anti-Group A carbohydrate tests. Because antibodies to the group A carbohydrate remain for several years in patients with persistent rheumatic valvular disease, this test should aid in the differentiation of rheumatic from non-rheumatic heart disease.

Background: Orthohantaviruses (also known as hantaviruses) are pathogens, primarily transmitted by rodents, that can cause hemorrhagic fever with renal syndrome (HFRS). In endemic regions of Kazakhstan, no confirmed HFRS cases were detected between 2020 and 2022 raising concerns about detection. Estimate antibody seroprevalence for hantaviruses and identify associated risk factors among high-risk adults in western Kazakhstan in 2023.

Here, we present a case of a 54-year-old complete remission acute myeloid leukemia patient, who was recently reported HIV negative by antibody-based tests used in National HIV testing programs.

A young male with no known addictions and comorbidities presenting with recurrent clonic-myoclonic movements, initially localized to the left corner of the mouth and left upper limb, evolving into epilepsia partialis continua, despite appropriate sequential antiepileptic medications, subsequently progressed to refractory status epilepticus. He was tested positive for HIV infection and his neuroimaging revealed nonenhancing lesions, a novel finding in HIV-related encephalitis. We managed him with intravenous immunoglobulin along with multiple antiepileptic medications and highly active antiretroviral therapy (ART), and he exhibited a rapid clinical recovery over 3 weeks.
