Objective: To describe renal tubular acidosis (RTA) and secondary acquired hyperaldosteronism in a cat as an adverse effect of topiramate therapy.

Case Summary: An 8-year-old neutered female cat on chronic oral topiramate therapy at a recommended dose (11.9 mg/kg q 8 h) for seizure control was presented with severe metabolic acidosis and hypokalemia. Plasma electrolyte and acid-base analysis identified a severe metabolic acidosis (pH 7.153, reference interval: 7.31-7.46), hypokalemia (2.08 mmol/L [2.08 mEq/L], reference interval: 3.5-4.8 mmol/L [3.5-4.8 mEq/L]), and ionized hypercalcemia (1.85 mmol/L [1.85 mEq/L], reference range: 1.1-1.4 mmol/L [1.1-1.4 mEq/L]). Urinalysis revealed a urine specific gravity of 1.021 and a pH of 7.0. Diagnostic workup suggested distal RTA as a cause of the cat's acid-base and electrolyte disturbances. Aldosterone concentration was moderately increased, suggestive of secondary hyperaldosteronism. The metabolic abnormalities resolved with supportive care and discontinuation of topiramate.

New Or Unique Information Provided: Topiramate is suggested to have led to the development severe RTA in a cat.

Prolonged use of antiretroviral agents has been clearly associated with nephrotoxicity, suggesting deterioration of renal function in patients receiving Highly Active Antiretroviral Therapy (HAART). The present study was designed to investigate the therapeutic efficacy of resveratrol (RV) in the treatment toxins-induced renal impairment. Twenty-four adult male Wistar rats weighing 70-90 g were divided into four groups and subjected to the following treatments: Control A (distilled water), B (HAART), C (RV-2.

N6-methyladenosine regulates metabolic remodeling in kidney aging through transcriptional regulator GLIS1.

Background: Age-related kidney impairment, characterized by tubular epithelial cell senescence and renal fibrosis, poses a significant global public health threat. Although N6-methyladenosine (m6A) methylation is implicated in various pathological processes, its regulatory mechanism in kidney aging remains unclear.

Methods: An m6A-mRNA epitranscriptomic microarray was performed to identify genes with abnormal m6A modifications in aged human kidney tissues.

Objective: A comprehensive bioinformatics analysis was conducted to investigate potential new diagnostic biomarkers and immune infiltration characteristics associated with tubulointerstitial injury in lupus nephritis (LN), and to examine possible correlations between key genes and infiltrating immune cells.

Methods: The GSE32591, GSE113342, and GSE200306 datasets were downloaded from the Gene Expression Omnibus database and differentially expressed genes (DEGs) were identified in the pooled dataset. Support vector machine-recursive feature elimination analysis and the least absolute shrinkage and selection operator regression model were used to screen for possible markers, and the compositional patterns of the 22 types of immune cell fractions in LN were determined using CIBERSORT.

Departments of Laboratory Medicine, Peking Union Medical College Hospital, Chinese Academy of Medical Sciences, Beijing 100730, China. Electronic address:

Background: To explore the underlying causes of significant fluctuations in creatinine levels within three days due to transient and severe uric acid elevation, and to provide evidence for the interpretation of abnormal test results and clinical diagnosis and management.

Methods: The issues were resolved by retesting the samples, comparing results across different detection platforms, and analyzing the reaction curve. We comprehensively reviewed patients' general conditions, imaging examinations, and treatments.
