Study Objectives: This pilot study aimed to investigate differences in the semantic meanings that individuals attribute to the words "sleepy," "fatigued," "tired," and "drowsy."
Methods: Ninety-six undergraduate students ranked the target words on 3 independent dimensions (evaluative, potency, and activity) to assess their meaning using the semantic differential technique. Participants also completed online questionnaires to assess their sleep difficulties and current states of sleepiness and fatigue.
Results: There were significant differences between all 4 words in connotative meaning on the evaluative dimension, < .05 for all post-hoc comparisons, with the differences largest between "sleepy" and "fatigued." "Drowsy" was significantly closer in meaning to "sleepy" than to "fatigued," = .04, and "tired" was not significantly closer in meaning to "sleepy" or "fatigued," = .13. No significant association was found between insomnia severity index scores and "sleepy" ratings, = 0.08, = .42, but a small effect was found with "fatigued" ratings, = 0.24, = .02.
Conclusions: These preliminary findings indicate that individuals consider these words as distinct concepts, relatively unaffected by the current severity of their insomnia symptoms. This adds to the argument for not using these words interchangeably, which we encourage sleep medicine researchers and clinicians to consider when employing these words in research and clinical practice.

Fatigue is a long-lasting problem in traumatic brain injury (TBI) and post-traumatic stress disorder (PTSD), with limited research that investigated the fatigue-related white-matter changes within TBI and/or PTSD cohorts. This exploratory cross-sectional study used diffusion tensor imaging (DTI) and neuropsychological data collected from 153 male Vietnam War veterans, as part of the Alzheimer's Disease Neuroimaging Initiative - Department of Defense, and were divided clinically into control veterans, PTSD, TBI, and with both TBI and PTSD (TBI + PTSD). Accidents caused by fatigue occur frequently, and numerous scholars have devoted tremendous efforts to investigate methods to reduce accidents caused by fatigued driving. Accordingly, the assessment of the spirit status of the driver through the eyes blinking frequency and the measurement of physiological signals have emerged as effective methods. In this study, a drowsiness detection system is proposed to combine the detection of LF/HF ratio from heart rate variability (HRV) of photoplethysmographic imaging (PPGI) and percentage of eyelid closure over the pupil over time (PERCLOS), and to utilize the advantages of both methods to improve the accuracy and robustness of drowsiness detection.
