Background: Hyaluronic acid fillers have been increasingly used throughout the world in the management of aging skin in the periocular region and other areas.
Objective: To evaluate very late-term orbital mass formation secondary to Hyaluronic acid filler injection into the inferior eyelid.
Methods: We report two cases that presented with a subcutaneous palpable mass localized at the right medial lower infraorbital area.

A cholesterol granuloma (CG) is characterized by the presence of cholesterol crystals that cause a chronic granulomatous reaction in an enclosed space. It occurs most commonly in the head and neck region, particularly in the middle ear. Although CGs in the maxilla have also been reported, odontogenic cysts in conjunction with CGs in the maxilla are very rare.

Infection with typically manifests as a self-limiting pulmonary disease in immunocompetent patients. Systemic symptoms such as cutaneous lesions are associated with immunodeficient states. Our patient was an immunocompetent 68-year-old male who presented with a plaque on his left infraorbital area that was concerning for malignancy.
