Spinal epidural abscess (SEA) is a rare but sometimes life-threatening condition. The principal organisms in SEA and spondylodiscitis are gram-positive bacteria, e.g., and . Spontaneous gram-negative SEA and spondylodiscitis especially are very rare. We report a 71-year-old Thai male with diabetes, presenting fever, enlarged neck mass, and progressive painful swallowing a week before admission. MRI of the whole spine demonstrated epidural abscess along the anterior thecal sac from C2 to C7 levels with spinal meningitis; multiple rim-enhancing lesions at the left sternocleidomastoid/levator scapulae, splenius capitis, semispinalis capitis, and bilateral scalene muscles; and rhombencephalitis with brain abscess. was isolated from blood culture. CT of the whole abdomen showed unremarkable intra-abdominal lesion. Intravenous ceftriaxone was administered, but the patient was unable to undergo surgical drainage due to unstable condition and died after two weeks of admission. Spontaneous SEA and spondylodiscitis caused by are very rare but sometimes fatal. In the case of SEA and spondylodiscitis, even when is uncommon, it should be also considered as a pathogen, especially when the patient had important risk factors.

Melioidosis rarely presents with spinal involvement, which may lead to neurological complications. It's endemic to Thailand and Australia, but rare in India. Patients with diabetes, immunocompromised states, and chronic renal failure are at high risk of developing melioidosis, which can present with localized or disseminated abscess collection.

Coronavirus disease 2019 (COVID-19) has been associated with numerous complications beyond the respiratory tract, including spinal and joint co-infections and secondary infections. However, we could not locate any reported cases of bilateral shoulder infections with additional spinal infections associated with mild (like our patient) COVID-19 or with more severe cases of COVID-19. We report the case of a healthy 62-year-old male who presented with mild COVID-19 and concurrent methicillin-sensitive (MSSA) bacteremia associated with multiple musculoskeletal sites of deep-seated infection.

Objective: The purpose of this study is to investigate the efficacy of the GAID-Protocol, a bundle of intra- and postoperative infection prevention measures, to reduce implant-associated infections in patients undergoing posterior spinal fusion with instrumentation. These preventive measures are organized into a protocol that includes recommendations for four critical areas of implant protection (acronym GAID): Gloves, Antiseptics: sodium hypochlorite/hypochlorous acid (NaOCl/HOCl), Implants and Drainage-use in large wounds.
Methods: We performed a single-site retrospective review of cases undergoing posterior spinal fusion with instrumentation for primarily degenerative spinal diseases before and after implementation of the GAID-Protocol that was specifically designed to protect against implant-associated infections.
