Background: Chordomas are malignant tumors that arise from the remnants of the notochord. Complete radical resection with postoperative radiation therapy is currently considered the gold standard. Here, we performed a 360-staged approach to manage a C3-C4 chordoma that involved the right vertebral artery.
Case Description: A 40-year-old woman presented with a C3-C4 chordoma that invaded the right vertebral artery. She responded well to a circumferential approach including resection and stabilization.
Conclusion: A 40-year-old woman with a C3-C4 spinal chordoma was optimally managed with a combined anterior/posterior surgical approach including decompression/fusion.

The first cervical vertebra (C1) is atypical in shape and bears a complex relationship with important neurovascular structures such as the vertebral artery and cervical spinal cord which are at risk of injury during misplaced screw fixation of C1. Placement of screws into the lateral mass of C1 vertebra is performed for stabilization of the craniovertebral junction. The objective of this study was to describe ideal screw dimensions, precise entry points, safe bony corridors, and ideal trajectories for placement of lateral mass screws in the Emirati population.

Introduction: Management of patients with large aortic arch aneurysms who are considered high risk for frozen elephant trunk technique have been challenging, especially when they have a dilated ascending aorta (AA) that precludes total endovascular branched repair (arch BEVAR). A viable option in our armamentarium is wrapping of the AA (AW), and zone 0 Ishimaru TEVAR.
Methods: Retrospective analysis of our aortic database from 2013 to 2024 to select high-risk patients with aortic arch aneurysm that had an AW and TEVAR.

They can occur spontaneously, even without any triggering factor. Classical clinical findings, such as headache or neck pain, may be absent, particularly in the context of a stroke.
