Background: Fetal ovarian cysts are the most frequently diagnosed intra-abdominal cysts; however, the evidence for perinatal management remains controversial.

Methods: We retrospectively reviewed cases of fetal ovarian cysts diagnosed by prenatal ultrasonography at our institution between January 2010 and January 2020. The following were investigated: gestational age at diagnosis, cyst size, appearance, prenatal ultrasound findings, and postnatal outcomes. Prior to 2018, expectant management was applied in all cases; after 2018, in utero aspiration (IUA) of simple cysts ≥40 mm was performed.

Results: We diagnosed 29 and seven simple and complex cysts, respectively. Fourteen patients had simple cysts with a maximum diameter <40 mm, and two of them progressed to complex cysts during follow-up; however, when the diameter was limited to <35 mm, no cases showed progression to complex cyst. Fifteen of the simple cysts were ≥40 mm; three progressed to complex cysts, and two of them were confirmed to be ovarian necrosis. In four patients who underwent IUA, the ovaries could be preserved.

Conclusions: IUA is a promising therapy for preserving ovaries with simple cysts ≥40 mm in diameter; however, the indications for fetal surgery and the appropriate timing of intervention require further study.

Study Question: Is there an association between dydrogesterone exposure during early pregnancy and the reporting of birth defects?

Summary Answer: This observational analysis based on global safety data showed an increased reporting of birth defects, mainly hypospadias and congenital heart defects (CHD), in pregnancies exposed to dydrogesterone, especially when comparing to progesterone.

What Is Known Already: Intravaginal administration of progesterone is the standard of care to overcome luteal phase progesterone deficiency induced by ovarian stimulation in ART. In recent years, randomized controlled clinical trials demonstrated that oral dydrogesterone was non-inferior for pregnancy rate at 12 weeks of gestation and could be an alternative to micronized vaginal progesterone.

Predicting factors of ovarian responses in infertile women with polycystic ovary syndrome undergoing IVF/ICSI.

J Assist Reprod Genet

January 2025

Department of Assisted Reproductive Medicine, Shanghai First Maternity and Infant Hospital, School of Medicine, Tongji University, 2699nd West Gao Ke Road, Shanghai, 201204, China.

Purpose: Women with polycystic ovary syndrome (PCOS) show greater heterogeneity in ovarian responses during ovarian stimulation. We aimed to investigate the potential predicting factors among individualized basic parameters that affect poor or hyper ovarian responses in PCOS patients.

Methods: We retrospectively screened 2058 women with PCOS who underwent their first cycle of in vitro fertilization/intracytoplasmic sperm injection.

Introduction: Acoustic shadowing is an important benign ultrasound (US) feature for adnexal masses (AMs). To validate the diagnostic performance and interobserver agreement of the 2019 version and 2022 version of Ovarian-Adnexal Reporting and Data System Ultrasound (O-RADS US) and ascertain whether adding acoustic shadowing to O-RADS US v2019 as a benign ultrasound feature can enhance its diagnostic efficacy among junior radiologist.

Methods: This retrospective study included consecutive women with suspected adnexal masses who underwent ultrasound examinations between September 2022 and January 2024.

This Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) systematic review synthesized effects of background levels of per- and polyfluorylalkyl substance (PFAS) levels on reproductive health outcomes in the general public: fertility, preterm birth, miscarriage, ovarian health, menstruation, menopause, sperm health, and in utero fetal growth. The inclusion criteria included original research (or primary) studies, human subjects, and investigation of outcomes of interest following non-occupational exposures. It drew from four databases (Web of Science, PubMed, Embase and Health and Environmental Research Online (HERO)) using a standardized search string for all studies published between 1 January 2017 and 13 April 2022.
