Emergence of NDM-1-Producing Nosocomial Isolates in Attica Region of Greece.

Microorganisms

August 2024

AMR and HAIs Laboratory, Central Public Health Laboratory, National Public Health Organization, Vari, 16672 Attica, Greece.

Here, we report on the emergence and spread of multidrug-resistant NDM-1-producing isolates from patients hospitalized in the Attica region, Greece, in 2022 to provide data on their resistome, their virulome, the genetic environment of , and their molecular epidemiology. A total of 17 carbapenem-resistant isolates identified as NDM-producers by immunochromatography at the hospital level were sent to the Central Public Health Laboratory, in the frame of the laboratory surveillance of carbapenem-resistant pathogens, for further characterization. The initial screening for genetic AMR determinants was carried out by PCR and the MDR Direct Flow Chip assay.

Objectives: We describe the clonal spread of New Delhi metallo-β-lactamase (NDM) 1-producing isolates belonging to the ST773 clone in Spain and the Netherlands, associated with the transfer of Ukrainian patients during the war.

Methods: Between March and December 2022, nine NDM-1-producing ST773 isolates were recovered from nine Ukrainian patients evacuated to two Spanish ( = 3) and five Dutch ( = 6) hospitals. Antimicrobial susceptibility testing was studied (Sensititre, Microscan, EUCAST-2023).

We report a nosocomial outbreak caused by a multidrug-resistant carbapenemase-producing Klebsiella pneumoniae (MDRCPKp), that was detected in six patients admitted to the medical intensive care unit between 20th of December 2023 and 15th of January 2024 in Ankara, Turkey. The investigation of this outbreak was started on 29th of December 2023. During the outbreak 11 samples were collected from the six patients with MDRCPKp.

Article Synopsis
  • A strain named fxq was isolated from a cerebrospinal fluid sample of a patient with tentorial meningioma, showcasing resistance to carbapenems due to the presence of KPC and NDM carbapenemase genes.
  • Identification methods included the Phoenix 100 system, MALDI-TOF, and whole-genome sequencing, revealing that fxq is a novel sequence type (ST202) with specific plasmid characteristics.
  • The study highlights the significant stability and transferability of the resistance plasmids, marking the first report of this particular resistant strain in a clinical setting.

Background: In the hospital environment, carbapenemase-producing Pseudomonas aeruginosa (CPPA) may lead to fatal patient infections. However, the transmission routes of CPPA often remain unknown. Therefore, this case study aimed to trace the origin of CPPA ST357, which caused a hospital-acquired pneumonia in a repatriated critically ill patient suffering from Guillain-Barré Syndrome in 2023.
