In 2012 several articles reported interesting findings for the ambulatory practice in internal general medicine. A negative rapid test for influenza does not rule out that diagnosis. A test assessing the walking speed in the elderly can help determining who would benefit from antihypertensive therapy.

Plasmin exposure modulates platelet aggregation responses, but a direct effect of plasmin on the platelet fibrinogen receptor, glycoprotein IIb/IIIa (GPIIb/IIIa), has never been conclusively shown in a plasma milieu. To examine this issue, we incubated platelets in platelet-rich plasma with plasmin and measured the effect of this treatment on platelet aggregation, fibrinogen binding, and the structural integrity of GPIIb/IIIa. Plasmin treatment reduced maximal reversible fibrinogen binding in a dose-dependent fashion, and this reduction in binding was accompanied by a correlative reduction in the maximal rate of aggregation.

Platelets

Thrombolytic therapy has galned increasing acceptance in the clinical arena in recent years, but is fraught with three primary problems: delays in time to lysis, reocclusion, and haemorrhage. Inasmuch as the platelet is a critical determinant of thrombosis and, at the same time, interacts with molecular elements important in fibrinolysis, this anuclear cell plays a central role in the outcome of thrombolytic therapy. Platelet-rich thrombi are less sensitive to thrombolysis than platelet-poor thrombi, and reocclusive thrombi are richer in platelets than are thrombi that form de novo.
