Hypothalamic neuropeptides named hypocretin/orexins which were identified in 1998 regulate critical functions such as wakefulness in the central nervous system. These past 20 years had revealed that orexins/receptors system was also present in the peripheral nervous system where they participated to the regulation of multiple functions including blood pressure regulation, intestinal motility, hormone secretion, lipolyze and reproduction functions. Associated to these peripheral functions, it was found that orexins and their receptors were involved in various diseases such as acute/chronic inflammation, metabolic syndrome and cancers. The present review suggests that orexins or the orexin neural circuitry represent potential therapeutic targets for the treatment of multiple pathologies related to inflammation including intestinal bowel disease, multiple sclerosis and septic shock, obesity and digestive cancers.

Melanonychia describes black pigmentation of the nail plate that results from either melanocyte activation (such as infections, local inflammatory disorders, local trauma affecting the nail plate, numerous systemic conditions, and medications) or melanocyte hyperplasia (such as benign neoplasms or malignant tumors) or blood (resulting from a trauma-associated subungual hematoma). The black dyschromia may include not only the nail plate but also the proximal nailfold. The Hutchinson sign refers to black discoloration of both the proximal nailfold and adjacent nail plate when the underlying pigmented lesion is a malignant melanoma.

The aim of our 5-year study (from January 2008 to December 2012) was to compare the incidence of peptic ulcer bleeding (PUB) to bleeding from tumors of the upper gastrointestinal tract. The percentage of re-bleeding within the first 30 days of hospital admission, as well as the need of blood transfusions, are also reported. Statistical data were collected on 2198 patients who were treated in our emergency department due to upper gastrointestinal bleeding (UGIB) in the form of melena or hematemesis.

Muir-Torre syndrome is a rare form of hereditary nonpolyposis colorectal cancer syndrome; simplified, it is an association of at least one sebaceous skin tumor and at least one visceral malignancy. It follows an autosomal dominant pattern of inheritance. We present a case of a recurrent sebaceous carcinoma of the eyelid in a patient previously operated on for colorectal carcinoma.
