Pulmonary arterial hypertension (PAH) is a disease of abnormal pulmonary vascular remodeling whose medical therapies are thought to primarily act as vasodilators but also may have effects on pulmonary vascular remodeling. The angiotensin II type 1 receptor (ATR) is a G protein-coupled receptor that promotes vasoconstriction through heterotrimeric G proteins but also signals via β-arrestins, which promote cardioprotective effects and vasodilation through promoting cell survival. We found that an AT1R β-arrestin-biased agonist promoted vascular remodeling and worsened PAH, suggesting that the primary benefit of current PAH therapies is through pulmonary vascular reverse remodeling in addition to their vasodilation.

In patients with suspected interstitial lung disease, diagnostic confirmation can be achieved through an awake video-assisted surgical lung biopsy. This procedure enables the collection of a substantial amount of parenchymal tissue for diagnostic purposes while minimizing perioperative complications associated with mechanical ventilation with the patient under general anaesthesia, given the impaired lung function due to the underlying condition.

Background: Calcific aortic valve disease is the pathological remodeling of valve leaflets. The initial steps in valve leaflet osteogenic reprogramming are not fully understood. Cystic cuboid adenomatous malformations (CCAM) are congenital pulmonary lesions, usually benign, that can progress into malignancy. Bronchopulmonary sequestration (BPS) is another type of malformation that consistsof an ectopic pulmonary tissue mass that doesn't participate in blood-gas exchanges, with vascularization provided by anomalous branches of the thoracic aorta. Hybrid lesions are lesions that have histological features of CCAM but with systemic vascularization, a pathognomonic sign of BPS.
