Background: The local flaps, especially perforator and keystone flaps, are used as first-line treatment option in reconstruction of small tomoderate-sized defect of the extremity. However, the high complication rate associated with these flaps may hinder this usage.
Methods: This article reviews the technical and clinical aspect of using color duplex ultrasound )CDU) in the preoperative, intraoperative, and postoperative period for propeller and keystone flaps.
Results: CDU allows the surgeon to understand the anatomical aspect of the perforator such as the location, point of penetration on the deep fascia, subcutaneous pathway )axiality) and physiological aspect such as velocity and flow volume. Understanding and utilizing this information will allow accurate preoperative design, intraoperative decision making, and postoperative monitoring, leading to better outcome.
Conclusion: Carefully designed local perforator flaps based on anatomy and physiology using CDU will be a powerful armamentarium for reconstruction of the lower extremity.

Lateral ankle soft tissue defects pose challenges, especially in cases due to chronic pressure from cross-legged sitting, which usually present with a large dead space, small skin opening that often accompanies an open joint. Traditional reconstruction methods using fasciocutaneous flaps may result in donor site morbidity such as delayed wound healing or nerve injury. In this article, we present a case of diabetes-related lateral ankle defect successfully treated using adiposal layer only flap, also known as pure fat flap.

Transcranial Doppler is an instrumental ultrasound method capable of providing data on various brain pathologies, in particular, the study of cerebral hemodynamics in stroke, quickly, economically, and with repeatability of the data themselves. However, literature reviews from clinical studies and clinical trials reported that it is an operator-dependent method, and the data can be influenced by external factors, such as noise, which may require greater standardization of the parameters. Artificial intelligence can be utilized on transcranial Doppler to increase the accuracy and precision of the data collected while decreasing operator dependencies.

Sulfonated indocyanines 3 and 5 (sCy3, sCy5) are widely used to label biomolecules. Their high molar absorption coefficients and lack of spectral overlap with biopolymers make them ideal as linker components for rapid assessment of bioconjugate stoichiometry. The effect of acetazolamide on regional brain tissue oxygenation in patients with acute brain injury (ABI) is unknown. We studied adult patients with ABI who received acetazolamide as per the treating physician's decision and had ICP and brain oxygen pressure (PbtO) monitoring. Baseline measurements of ICP, cerebral perfusion pressure (CPP), and PbtO were taken before administering acetazolamide; subsequent measurements were recorded every 5 min for a total of 20 min.
