Novel psychoactive substances have emerged as drugs of abuse. 2-Fluorodeschloroketamine (2-FDCK) is a ketamine derivative that can be purchased online for as little as $12 per gram. We report the case of a patient with a history of polysubstance use presenting after insufflation of 2-FDCK, with subsequent confirmation of metabolites in the patient's urine. A 28-year-old man presented to the Emergency Department in a dissociated state. He recovered well with supportive care, and described during interview his experience of substance use including the novel psychoactive substance 2-FDCK. A urine sample was sent for analysis and 2-FDCK metabolites were isolated. This case is concerning because 2-FDCK is a relatively new agent that has not yet been reported in the United States. It is easy to obtain over the internet and has significant abuse potential.

Background: Tramadol is an adulterant of illicit opioids. As it is a serotonin-norepinephrine reuptake inhibitor as well as a μ-opioid agonist, tramadol adulteration may worsen overdose signs and symptoms or affect the amount of naloxone patients receive.

Methods: This is a multicenter, prospective cohort of adult patients with suspected opioid overdoses who presented to one of eight United States emergency departments and were included in the Toxicology Investigators Consortium's Fentalog Study.

Although several studies have well described the characteristics of people who use psychedelics alongside their motivations and beliefs, little research has examined the preferences surrounding the source of psychedelic substances. In an anonymous online survey, we collected data from 6,379 consumers of 11 different psychedelic substances from 85 different countries, exploring their preferences and perceptions on natural and synthetic psychedelics. There was a strong preference of natural sources over synthetic alternatives for psilocybin (75%), DMT (56%), and mescaline (56%).

Introduction: Psychedelic compounds such as LSD, psilocybin, mescaline, and DMT can dramatically alter visual perception. However, the extent to which visual effects of psychedelics consistently vary for different substances is an open question. The visual effects of a given psychedelic compound can range widely both across and within individuals, so datasets with large numbers of participants and descriptions of qualitative effects are required to adequately address this question with the necessary sensitivity.
