Sleep-Disordered Breathing and Cardiac Arrhythmias.

J Clin Med

November 2024

Division of Cardiology, Department of Medicine, University of Louisville School of Medicine, Louisville, KY 40202, USA.

Article Synopsis
  • A narrative review analyzed peer-reviewed studies to explore how sleep-disordered breathing (SDB) links to cardiac arrhythmias through factors like intermittent hypoxia and autonomic imbalance.
  • Patients with SDB face significantly higher risks for various heart rhythm issues, with more than double the chance of overall arrhythmias and a quadrupled risk of atrial fibrillation (AF).
  • CPAP therapy is shown to be an effective treatment, reducing AF recurrence and lowering the risk of ventricular arrhythmias, emphasizing the need for early diagnosis and management of SDB to improve heart health.

Background: Postextubation respiratory failure (PRF) frequently complicates weaning from mechanical ventilation and may increase morbidity/mortality. Noninvasive ventilation (NIV) alternating with high-flow nasal oxygen (HFNO) may prevent PRF.

Methods: Ventilated patients without chronic obstructive pulmonary disease (COPD) and at high-risk of PRF defined as a lung ultrasound score (LUS) ≥ 14 assessed during the spontaneous breathing trial, were included in a French-Chinese randomised controlled trial.

Objective: Positive airway pressure (PAP) compliance is important in treating obstructive sleep apnea. Previous studies have suggested that patients with economic burdens, discomfort using machines, and insufficient education have difficulty tolerating PAP machines. This study explored the factors affecting short-term adherence to PAP in a veterans medical center.

Multiple states have enacted statutes to govern procedures when a state seeks to execute a person who may be incompetent to understand why s/he is being so punished, an area of the law that has always been riddled with confusion. The Supreme Court, in , sought to clarify matters, ruling that a mentally ill defendant had a constitutional right to make a showing that his mental illness "obstruct[ed] a rational understanding of the State's reason for his execution."However, the first empirical studies of how has been interpreted in federal courts painted a dismal picture.

Background: Inflammation plays a central role in the genesis and progression of heart failure with preserved ejection fraction (HFpEF). C-reactive protein (CRP) is widely used as means to assess systemic inflammation, and elevated levels of CRP have been associated with poor HF prognosis. Identification of chronic low-grade inflammation in outpatients can be performed measuring high-sensitivity CRP (hsCRP).
