Melioidosis is a disease caused by . Highly endemic areas include tropical Australia and Southeast Asia, though cases have been reported in the Americas. To our knowledge this is the first case to have occurred due to presumed exposure in Nicaragua, demonstrating the need for increased awareness. In addition, the severity of melioidosis also varies widely and more research is needed on the pattern of disease particularly in non-endemic regions.

Burkholderia pseudomallei, the causative agent of melioidosis, is intrinsically resistant to multiple classes of antibiotics and primarily affects immunocompromised individuals, such as those with poorly controlled diabetes or malignancies. In this case, a 58-y-old female farmer with poorly controlled diabetes (HbA1c of 11.4%), metastatic breast cancer with chemotherapy-induced pancytopenia and disseminated melioidosis showed no improvement despite receiving antibiotics and supportive care.

Background: Although there is a high incidence of hematogenous infections in melioidosis, a tropical infectious disease, there are few systematic analyses of hematogenous melioidosis in imaging articles. This case report details the clinical course of a 53-year-old male farmer with a 15-year history of diabetes mellitus who presented with a 20-day history of pyrexia, rigors, and shivering, as well as problems in the urogenital system and left hypochondrial pain. Notably, he had been diagnosed with spinal tuberculosis, which was successfully treated five years ago. On evaluation, there was tenderness in the suprapubic region as well as the left hypochondrium; moreover, rectal examination showed that the prostate was boggy and tender.
