Reel's syndrome (RS) is an unusual cause of pacemaker lead dislodgement. We present the case of a 59-year-old female patient with Down syndrome (DS) implanted with a dual-chamber endovascular pacemaker due to symptomatic sinus node disfunction, reporting several syncopal episodes in last days and showing abnormal electrical parameters at the 2-months follow-up due to RS. The malfunctioning device was removed and an endocardial leadless pacing system was implanted.

Introduction: cardiac pacing is the only lifesaving procedure which is effective for major cardiac conduction disorders. In sub-Saharan Africa, few pacemakers are implanted, compared to Western countries. This study aimed to describe the indications for cardiac pacing in four hospitals in Senegal, to evaluate its practical modalities, to identify pacemaker's complications and their predisposing factors and to evaluate the main challenges for cardiac pacing in Senegal.

Silent Brain Infarction (SBI) is increasingly recognized in patients with cardiac conditions, particularly Atrial Fibrillation (AF) in elderly patients and those undergoing Transcatheter Aortic Valve Implantation (TAVI). While these infarcts often go unnoticed due to a lack of acute symptoms, they are associated with a threefold increase in stroke risk and are considered a precursor to ischemic stroke. Moreover, accumulating evidence suggests that SBI may contribute to the development of dementia, depression, and cognitive decline, particularly in the elderly population.
