Clubfoot overcorrection can develop gradually over years and is characterized in school age by hindfoot valgus position due to excessive rigid peritalar eversion, sometimes accompanied by supramalleolar valgus malalignment. Surgical treatment is recommended in severe cases and consists of bony realignment at the peritalar complex by osteotomy or fusion, correction of the supramalleolar valgus deformity in younger children by hemiepiphyseodesis, or osteotomy in adolescents. In addition, dorsal bunion requires stabilization of the medial tarsometatarsal ray and transfer of the tendons of Mm. tibialis anterior and flexor hallucis longus.

Background: The aim of this study was to investigate the clinical and radiologic outcomes following supramalleolar osteotomy for early varus ankle arthritis with the medial translation of the talus, wherein the lateral translation of the talus center to the tibial axis was used as a correction target. Another aim was to compare the results between the ankles with a normal range of medial distal tibial angle (MDTA) and overcorrected MDTA at the postoperative period to investigate the role of overcorrected MDTA after the supramalleolar osteotomy.

Methods: The study comprised 90 patients (93 ankles) with Takakura stage 2 or 3a varus ankle arthritis with the medial translation of the talus, which underwent medial opening supramalleolar osteotomy with fibular osteotomy, and the mean duration of follow-up was 50.

Supramalleolar osteotomy (SMO) aims to correct extra-articular deformities of the distal lower leg. There are several indications, the most common being varus osteoarthritis of the ankle. The rationale in this indication is to modify talocrural stress and pressure distribution by reorienting the limb axis.

Article Synopsis
  • A study looked at how surgery can help improve knee problems in adults who had previous tibial plateau fractures that didn’t heal correctly.
  • Seven patients were treated with different types of surgery to fix their knee angles, and most of them had better knee movement afterward.
  • The average recovery time to bear weight on the knee again was about 81.5 days, and some patients might need a more serious knee surgery later on.

Joint preserving procedures for Takakura stage IIIB varus ankle osteoarthritis.

BMJ Case Rep

Department of Orthopaedic Surgery, Inje University Ilsan Paik Hospital, Goyang-si, Gyeonggi-do, Korea (the Republic of).

A man in his early 70s with right Takakura stage IIIB varus ankle osteoarthritis underwent medial opening wedge supramalleolar osteotomy with inframalleolar correction as joint preserving procedure. We also performed anteroinferior tibiofibular ligament (AITFL) resection with fibular shortening valgisation osteotomy to enhance the talar tilt correction. Postoperative decrease in talar tilt with dramatic symptom improvement was achieved.

A finite element model of human hindfoot and its application in supramalleolar osteotomy.

Background: The majority of the ankle osteoarthritis cases are posttraumatic and affect younger patients with a longer projected life span. Hence, joint-preserving surgery, such as supramalleolar osteotomy becomes popular among young patients, especially those with asymmetric arthritis due to alignment deformities. However, there is a lack of biomechanical studies on postoperative evaluation of stress at ankle joints.
