Case: An osteoporotic and kyphotic 72-year-old woman presented with sternal pain, severe chest instability, and shortness of breath secondary to a traumatic sternal nonunion resulting from an unhealed sternal fracture sustained in a motor vehicle accident several months earlier. One-year functional and radiographic outcomes are presented with complete resolution of symptoms.
Conclusion: Consideration for treatment with multiplanar, contourable, locked plating, augmented by autogenous graft, is warranted in the setting of sternal deformity and nonunion, particularly in the setting of osteoporosis and other sagittal plane deformity.

Objective: This retrospective cohort study aimed to evaluate the effectiveness of posterior long-segment fixation for thoracolumbar osteoporotic vertebral compression fractures (TLOVCFs) and identify prognosis-predicting factors.
Methods: Twenty-two patients with TLOVCFs who underwent posterior long-segment fixation between January 2013 and June 2022 were included. Preoperative, postoperative, and final follow-up data were collected, including demographics, bone mineral density, Cobb angle (CA) measurements, and neurological outcomes based on the Frankel classification.

The osteoporotic fracture treatment score (OF score), developed by the German Society of Orthopedics and Trauma (DGOU), guides surgical decisions, but its reliability and validity are underexplored. Background Context: Percutaneous vertebroplasty for the treatment of osteoporotic burst fractures remains controversial. A previous study has shown that an osteoporotic burst fracture with asymptomatic spinal canal compromise is not a contraindication for percutaneous vertebroplasty.
