Background: Placement of an intercostal chest drain (ICD) is an essential procedure in the management of patients with chest injuries. However, ICD placement can have complications. Here, we report a case of massive hemothorax due to injury of an anonymous vein associated with ICD placement.

Case Presentation: An 84-year-old man with chronic right pleural effusion from pleuroperitoneal communication presented with dyspnea after a fall. An ICD was placed in the right seventh intercostal area on the middle axillary line. He later complained of chest pain and dyspnea again due to right pneumothorax, and massive hemorrhagic pleural effusion was drained from an additionally placed ICD. A contrast-enhanced computed tomography scan showed that bleeding from the parietal pleura traveled along the first ICD and dropped into the intrapleural space. Intraoperatively, there was intramuscular venule bleeding from the right serratus anterior muscle, which was then ligated to stop the bleeding.

Discussion: An optimal area to place an ICD is termed the "safety triangle", which is determined by the pectoralis major, latissimus dorsi, and the level of the nipples and the base of the axilla. In this case, the ICD was placed in the seventh intercostal area, which is more than two intercostal distances inferior to the 'safety triangle'.

Conclusions: This case suggested that, even though the vessel was small, a massive, life-threatening hemothorax can occur if an injury is caused by ICD placement. Knowledge of the anatomy necessary for placing an ICD should be reinforced.

This study aimed to assess measurement invariance for the Five-Factor Inventory for (Oltmanns & Widiger, 2020) across nine national samples from four continents ( = 6,342), and to validate a French translation in seven French-speaking national samples. All were convenience samples of adults. Exploratory factor analyses supported a four-factor structure in the French-speaking Western samples (Belgium, Canada, France, and Switzerland) while a three-factor structure was preferred in the French-speaking African samples (Burkina Faso and Togo), and no adequate structure was found in the Indian sample.

Background: Clear cell renal cell carcinoma (ccRCC) remains a challenging cancer type due to its resistance to standard treatments. Immunogenic cell death (ICD) has the potential to activate anti-tumor immunity, presenting a promising avenue for ccRCC therapies.

Methods: We analyzed data from GSE29609, TCGA-KIRC, and GSE159115 to identify ICD-related prognostic genes in ccRCC.

Background: The immune response to infections may become dysregulated and promote myocardial damage contributing to heart failure (HF). We examined the relationship between infection-related hospitalization (IRH) and HF, HF with preserved ejection fraction, and HF with reduced ejection fraction.

Methods And Results: We studied 14 468 adults aged 45 to 64 years in the ARIC (Atherosclerosis Risk in Communities) Study who were HF free at visit 1 (1987-1989).

Cardiac sarcoidosis; update for the heart failure specialist.

Curr Opin Cardiol

January 2025

Division of Cardiology, University of Ottawa Heart Institute, University of Ottawa, Faculty of Medicine, Tier 1 Clinical Research Chair in Cardiac Electrophysiology, Ottawa, ON, Canada.

Purpose Of Review: This review presents contemporary data on epidemiology, common presentations, investigations and diagnostic algorithms, treatment and prognosis. It particularly focuses on topics of most relevance to heart failure specialists, including what left ventricle (LV) function changes can be expected after treatment and outcomes to all standard and advanced heart failure therapies.

Recent Findings: Around 5% of sarcoidosis patients have clinically manifest cardiac sarcoidosis (CS), presenting with significant arrhythmias (such as conduction disturbances and ventricular arrhythmias) or newly developed unexplained heart failure.

Associations of Traumatic Brain Injury and Mild Behavioral Impairment With Cognitive Function and Dementia.

J Geriatr Psychiatry Neurol

January 2025

Division of Geriatric Psychiatry and Neuropsychiatry, Department of Psychiatry and Behavioral Sciences, Johns Hopkins University School of Medicine, Baltimore, MD, USA.

Objective: Traumatic Brain Injury (TBI) may contribute additional complexity to the clinical picture of mild behavioral impairment (MBI). MBI, a behavioral analog to mild cognitive impairment (MCI), is comprised of five neuropsychiatric domains: decreased motivation, affective dysregulation, impulse dyscontrol, social inappropriateness, and abnormal perception/thought content. We investigated (1) if cross-sectional associations of cognitive status with MBI symptoms differ by TBI status and (2) if prospective associations of MBI domain positivity with incident dementia risk differ by TBI status.
