Oh wait … It isn't MUDPILES! Acute toxic encephalopathy with an interesting anion gap metabolic acidosis resulting in prolonged invasive mechanical ventilation.

Internal Medicine Division, White River Health System, Batesville, AR, USA.

Published: September 2021

A 70-year-old white female patient with past medical history of migraine, fibromyalgia, diverticulitis, and hypothyroidism presented to the emergency department accompanied by her husband for one day of altered mentation, nausea and vomiting. Laboratory testing showed oligo-anuric acute kidney injury with a severely high anion gap metabolic acidosis. Urine drug screen was negative. Brain imaging and lumbar puncture were negative for acute findings. We report this unique case by going through the differential for anion gap metabolic acidosis secondary to Celecoxib as well as a unique drug-drug interaction between Celecoxib and Gabapentin.

Distal renal tubular acidosis (dRTA) is a significant clinical expression of Sjögren's syndrome (SS). While SS-related dRTA is traditionally linked to impaired H-ATPase, we report a unique case demonstrating selectively decreased anion exchanger 1 (AE1) expression with preserved H-ATPase expression. A 16-year-old girl with SS presented with muscle weakness, difficulty in ambulation, and severe hypokalemia.

A Case of Euglycemic Ketoacidosis Secondary to Continuous Renal Replacement Therapy.

Case Rep Crit Care

January 2025

Division of Pulmonary, Critical Care & Sleep Medicine, Keck Hospital of USC, Los Angeles, California, USA.

Euglycemic ketoacidosis (EKA) has been reported as a rare but life-threatening complication of continuous renal replacement therapy (CRRT). EKA should be suspected in the setting of persistent high anion gap metabolic acidosis despite renal replacement therapy. Critically ill patients, especially those with diabetes mellitus, are at risk of EKA due to deficient caloric intake, the presence of excess counterregulatory stress hormones, and nutritional losses from CRRT.

Purpose: Perioperative metabolic acidosis negatively affects patient outcomes. Perioperative fluid therapy has a clinically significant effect on acid-base balance. This study was conducted to evaluate the effects of isotonic sodium bicarbonate infusion (ISB) versus balanced crystalloid solution (BCS) on perioperative acid-base balance, in terms of postoperative base excess, among patients undergoing emergency laparotomy for perforation peritonitis.

With over 14 million people living above 3,500 m, the study of acclimatization and adaptation to high altitude in human populations is of increasing importance, where exposure to high altitude (HA) imposes a blood oxygenation and acid-base challenge. A sustained and augmented hypoxic ventilatory response protects oxygenation through ventilatory acclimatization, but elicits hypocapnia and respiratory alkalosis. A subsequent renally mediated compensatory metabolic acidosis corrects pH toward baseline values, with a high degree of interindividual variability.
