rt-PA for ischemic stroke in the unknown or extended time window beyond the first 4. 5 h after symptom onset is safe and effective for certain patients after selection by multimodal neuroimaging. However, the evidence for this approach comes mainly from patients with anterior circulation stroke (ACS), while the data on posterior circulation stroke (PCS) are scarce. Ischemic stroke patients treated with IV-thrombolysis in the unknown or extended time window between January 2011 and May 2019 were identified from an institutional registry. The patients were categorized into PCS or ACS based on clinico-radiological findings. We analyzed the hemorrhagic complications, clinical and imaging efficacy outcomes, and mortality rates by comparing the PCS and ACS patient groups. Adjusted outcome analyses were performed after propensity score matching for the relevant factors. Of the 182 patients included, 38 (20.9%) had PCS and 144 (79.1%) had ACS. Symptomatic acute large vessel occlusion (LVO) was present in 123 patients on admission [27 (22.0%) PCS and 96 (78.0%) ACS]. The score on the National Institutes of Health Stroke Scale (NIHSS), the time from last seen normal, and the door-to-needle times were similar in PCS and ACS. In patients with LVO, the NIHSS score was lower [8 (5-15) vs. 14 (9-18), = 0.005], and infarction visible on follow-up imaging was less common [70.4 vs. 87.5%; aRD, -18.9% (-39.8 to -2.2%)] in the PCS patient group. There was a trend toward a lower risk for intracranial hemorrhage (ICH) following intravenous thrombolysis in PCS vs. ACS, without reaching a statistical significance [5.3 vs. 16.9%; aRD, -10.4% (-20.4 to 4.0%)]. The incidence of symptomatic ICH [according to the ECASS III criteria: 2.6 vs. 3.5%; aRD, -2.9% (-10.3 to 9.2%)], efficacy outcomes, and mortality rates were similar in PCS and ACS patients. In this real-world clinical cohort, the safety and the efficacy of rt-PA for ischemic stroke in the unknown or extended time window did not show relevant differences between PCS and ACS, with a trend toward less hemorrhagic complications in PCS. The findings reconfirm the clinician in the usage of rt-PA beyond the first 4.5 h also in selected patients with PCS.

Argatroban plus alteplase in posterior versus anterior circulation stroke.

Ann Clin Transl Neurol

December 2024

Department of Neurology, General Hospital of Northern Theater Command, No. 83 Wenhua Road, Shenhe District, Shenyang, 110016, China.

Objective: ARAIS trial failed to demonstrate benefit of argatroban as an adjunct to alteplase for stroke. Given differences between anterior circulation stroke (ACS) and posterior circulation stroke (PCS), we performed prespecified secondary analysis to investigate whether benefit of argatroban was different between ACS and PCS.

Methods: In secondary analysis of ARAIS, patients with definite stroke territories based on responsible vessel examination were classified into ACS and PCS.

Objective: The Antiplatelet versus R-tPA for Acute Mild Ischaemic Stroke trial has demonstrated the non-inferiority of dual antiplatelet therapy (DAPT) to alteplase in minor non-disabling stroke. This prespecified secondary analysis aimed to investigate whether the treatment effects were similar across stroke territories.

Methods: Participants were divided according to stroke territory, which were subdivided into DAPT and alteplase.

Clearance of Protein-Bound Uremic Toxins Using Anion Nanotraps with Record High Uptake.

ACS Appl Mater Interfaces

December 2024

College of Chemical and Biological Engineering, Zhejiang University, Hangzhou 310058, China.

Article Synopsis
  • Traditional hemodialysis is often ineffective at removing protein-bound uremic toxins (PBUTs) like -cresyl sulfate and indoxyl sulfate due to their strong attachment to serum albumin, which can lead to cardiovascular issues.
  • A new class of cationic polymeric networks, CPN-X6-CPN-X9, shows exceptional capabilities in removing these toxins, with CPN-X7 and CPN-X6 achieving maximum sorption capacities that surpass any previously known materials.
  • CPN-X9, which is hydrophobic, displays impressive selectivity in experiments and is able to recycle effectively over five cycles while maintaining good compatibility with blood.

Fabrication of PBAT Membranes with High Separation Accuracy Based on Inverse-Opal-Structural Photonic Crystals.

ACS Appl Mater Interfaces

December 2024

Zhejiang Provincial Key Laboratory of Fiber Materials and Manufacturing Technology, Zhejiang Sci-Tech University, Hangzhou 310018, People's Republic of China.

Membrane technology is one of the most effective ways to cope with the filtration and separation. However, most polymer membranes are not uniform in pore size and have wide pore size distributions and low porosities, which restrict their application. A high-porosity homogeneous membrane with high filtration accuracy is an effective solution.

Article Synopsis
  • The study investigates how nanoplastics (NPs) interact with the human body, particularly concerning their roles in cancer, revealing uncertainties in how NPs affect tumor behavior.
  • Analyzing protein coronas formed on NPs from thyroid cancer patients showed an increase in fibrinogen, which accelerated the uptake of NPs by cancer cells compared to healthy individuals.
  • Alterations in protein coronas due to clinical conditions were found to enhance NPs engulfment by macrophages, leading to pro-inflammatory responses that may promote tumor growth, highlighting the need to understand NPs' biological interactions.
