Refractory seizures are common in pediatric neurology; consideration of whether seizures are acutely symptomatic of an underlying disease process is critical. In this case, a previously healthy 7-year-old patient presented with intractable focal seizures in the setting of a viral prodrome and headache. This case highlights a comprehensive work up of new-onset refractory seizures in a patient with features of encephalitis and describes a diagnostic quandary that ultimately led to a rare unifying diagnosis.

The leucine-rich glioma-inactivated protein 1 (LGI1) antibody-related autoimmune encephalitis can occur alone or in the setting of a malignancy and manifest with faciobrachial dystonic seizures (FBDS), cognitive decline, hyponatremia, and neuropsychiatric disorders. The importance of differentiating this entity from acute delirium cannot be overemphasized. This review provides a detailed account of a 71-year-old man with previous diagnosis of lung cancer who presented with subacute onset behavioural changes, urinary retention, and FBDS.

Autoimmune hemolytic anemia is a disorder that is characterized by the destruction of red blood cells through an autoimmune process, such as temperature-dependent antibodies. The two predominant types, cold agglutinin and warm agglutinin disease, typically possess different underlying etiologies. Prompt recognition and workup of autoimmune hemolytic anemia should be prioritized to potentially uncover any underlying primary cause, such as malignancy.
