AI Article Synopsis

  • Valve-in-valve (ViV) TAVI is a treatment option for patients with aortic bioprosthetic valve degeneration, but it carries a risk of coronary occlusion.
  • The case presented involves using a double chimney technique to safeguard the coronary ostia during the procedure.
  • After the initial valve placement, a "three-kissing balloon" approach was employed for post-dilation to reduce high residual transvalvular gradient.

Article Abstract

Valve-in-valve (ViV) transcatheter aortic valve implantation (TAVI) is an effective treatment for aortic bioprosthetic valve degeneration. ViV-TAVI could lead to coronary occlusion. We describe the case of a patient treated with double chimney technique to protect coronary ostia followed by post-dilation for high residual transvalvular gradient using "three-kissing balloon" approach. Background: Mitral annular calcification (MAC) is common in the elderly. Extensive calcification has been historically challenging for the cardiac surgeons, with traditional surgical approaches carrying significant risks. Less invasive approaches have recently been explored in an attempt to reduce this risk.

Article Synopsis
  • The study aimed to assess how double chimney thoracic endovascular aortic repair (ch-TEVAR) affects gutter phenomena and endograft deformations in a model that simulates real blood flow conditions.
  • Researchers compared two types of chimney grafts (balloon-expandable and self-expandable) using advanced imaging techniques to evaluate their behavior during different phases of the heart cycle.
  • Results showed that balloon-expandable grafts had significantly larger gutter volumes and exhibited different deformation patterns than self-expandable grafts, highlighting important design considerations for future devices to mitigate risks like endoleak.

Background: The aim of this study was to evaluate the early results of the CE-marked standardized device combination consisting of Endurant and the Radiant chimney graft (En-ChEVAR) for the treatment of juxtarenal aortic aneurysms.

Methods: We analyzed multicentric non-industry sponsored case series evaluating the EnChEVAR technique for patients treated between December 2022 and February 2024. Clinical, perioperative procedure-related and radiological data were collected.

The outcomes of redo mitral valve replacement (Re-MVR) in a small mitral annulus with the use of the chimney technique are not well documented. The purpose of this study is to present our early experience with this group of patients, illustrating the periop-erative complications and mortality outcomes. From 2019 to 2020, 77 consecutive patients underwent Re-MVR with the use of the chimney technique because of a small mitral annulus.
