Cytokine release syndrome (CRS) is an infrequently described immune-related adverse event of checkpoint inhibitors (CPI). CPI-induced CRS typically presents with fevers, hemodynamic instability and organ dysfunction within 2 weeks of the last treatment cycle. We report an unusual case of delayed and severe CRS occurring postoperatively in a patient with hepatic-limited metastatic colorectal cancer who received neoadjuvant immunotherapy. After a negative workup for alternative causes, he received prolonged corticosteroid treatment with symptom resolution. CPI-induced CRS can mimic sepsis and clinicians should maintain a high-index of suspicion to diagnose this immune-related adverse event early and initiate appropriate treatment. As use of perioperative immunotherapy increases, the potential role of surgery to trigger CRS in this case warrants further investigation.

Background: CAR T-cell therapy (CAR-T) is leading to durable responses in patients with cancer but there is concern that cytokine release syndrome (CRS) and neurotoxicity may impact survivors' cognitive function. We assessed long-term cognitive function in CAR-T recipients and examine factors associated with change in cognition over time.
Methods: We assessed perceived cognition (Functional Assessment of Cancer Therapy - Cognition) and neurocognitive performance (standardized neuropsychological battery) in adult patients prior to receiving CAR-T and at 6 month follow-up.

Cigarette smoke extract (CSE)-induced airway mucus hypersecretion and inflammation are prominent features of chronic obstructive pulmonary disease (COPD). The potent pro-inflammatory cytokine, interferon gamma (IFN-γ), is an enticing therapeutic target because of its accelerator role in several acute and chronic inflammatory processes. In this work, poloxamer 407 is developed as an in-situ gelling polymer for a long-acting formulation to deliver a serine protease, C5a peptidase (ScpA) from Streptococcus pyogenes. ScpA is well known for its activity against the complement factor C5a but has also recently been shown to cleave IFN-γ in vitro into inactive fragments.
