Thick-walled Gallbladder: A Pragmatic Management Approach.

Euroasian J Hepatogastroenterol

December 2024

Department of General and Minimal Invasive Surgery, Sher-I-Kashmir Institute of Medical Sciences Soura, Srinagar, Jammu and Kashmir, India.

Introduction: Thick-walled gallbladder (TWGB) is a common yet non-specific radiological finding associated with a wide range of gallbladder pathologies, including acute and chronic inflammation, infection, and malignancy. Among the inflammatory causes, xanthogranulomatous cholecystitis (XGC) is a rare but significant condition that often mimics gallbladder carcinoma. This paper presents a pragmatic approach to the diagnosis and management of TWGB, focusing on the complexities posed by XGC.

Article Synopsis
  • Porcelain gallbladder is a rare condition marked by calcification of the gallbladder wall, increasing the risk of gallbladder cancer and leading to high levels of the tumor marker CA19-9.
  • A case study presented a 60-year-old woman with abdominal pain and nausea, where imaging revealed porcelain gallbladder and common bile duct stones; her CA19-9 levels were extremely elevated after stone removal.
  • Post-surgery, histopathology showed no cancer but inflammation and calcification, and CA19-9 levels normalized, emphasizing that benign conditions can elevate CA19-9 and the importance of accurate diagnosis.

Introduction: Laparoscopic simple nephrectomy (LSNx) is among the gold standard options for management of selected poorly functioning and most non-functioning kidneys (PFK/NFKs). Our objective was to audit our patients who had undergone LSNx for benign causes and assess their perioperative outcomes and analyse the factors correlating with these outcomes.

Materials And Methods: We reviewed our prospectively collected data of patients with PFK/NFKs due to benign causes who underwent a LSNx on a three-dimensional (3D) system via a transperitoneal approach between January 2017 and June 2023.

Stage III xanthogranulomatous pyelonephritis with sarcomatoid degeneration.

Radiol Case Rep

December 2024

Department of Radiology, Ospedale del Mare, ASL NA1 Centro, Naples, Italy.

Article Synopsis
  • Xanthogranulomatous pyelonephritis (XGP) is a rare chronic kidney condition linked to factors like kidney stones, diabetes, and recurrent infections, with a potential risk of cancer in up to 11% of cases.
  • A 76-year-old woman presented with abdominal and back pain; imaging revealed a nonfunctioning kidney with necrotic areas and a nephron-cutaneous fistula, indicating advanced XGP.
  • This case highlights the importance of using multiple imaging methods, such as MDCT and contrast-enhanced ultrasound, for accurately diagnosing and managing patients with XGP, especially when suspicious masses are found.
