Shared decision making in pediatric dermatology: context, opportunities, and practical examples.

Purpose Of Review: Shared decision making (SDM) is an important part of patient-centered care. However, it is neither widely practiced nor researched in pediatric dermatology. In this article, we provide practical examples of how to engage in SDM in pediatric dermatology, and identify future areas of research.

Recent Findings: Children and parents/guardians desire SDM in clinical encounters. The process is applicable to discussions of medical as well as surgical care. Additionally, SDM can help prepare children for the transition from pediatric to adult/general providers. Clinicians often want more guidance on its implementation, and there is a dearth of research on SDM or decision tools specific to pediatric dermatology.

Summary: SDM is underused and understudied in pediatric dermatology. This article highlights how to engage in SDM and presents opportunities for research and implementation in pediatric dermatology.

Hydroa vacciniforme lymphoproliferative disorders (HVLPD) fall within the clinical spectrum of chronic active epstein barr virus (EBV) disease (CAEBVD), ranging from localised and/or indolent forms (classic HVLPD) to systemic disease with fever, hepatosplenomegaly and lymphadenopathy (systemic HVLPD). A preadolescent male with 47XYY, multicystic dysplastic kidney, autism spectrum disorder and Attention-deficit/hyperactivity disorder (ADHD) presented with photodistributed non-pruritic, non-painful necrotic papulovesicles accompanied by non-febrile intermittent fatigue and lymphadenopathy. The patient had a history of EBV pneumonia in infancy confirmed by CT scan and was later diagnosed with CAEBV.

Vulvovaginal enterobiasis is underreported and an often-missed diagnosis in prepubertal girls presenting with isolated nocturnal vulvar pruritus. An 8-year-old-girl with intense nocturnal vulvar itching for 4 months was diagnosed with enterobiasis after microscopy of a vaginal swab revealed Enterobius vermicularis (pinworm) ova. The child and her family were treated with two doses of oral albendazole, 2 weeks apart, resulting in symptom resolution.

Affected hair shafts are triangular or kidney-shaped with longitudinal grooving.
