• Persistent EV may be associated with thrombosis. • EV thrombi can present as asymptomatic right atrial mass. • Transesophageal echocardiography is useful to identify site and size of EV thrombi. • Surgical excision is diagnostic and therapeutic. • Patients with EV thrombi may benefit from long-term anticoagulation.
