Although implantable cardioverter-defibrillators (ICDs) and quinidine are primary treatments, recurrent BrS-triggered ventricular arrhythmias can persist. A thrombus straddling a patent foramen ovale (TSPFO) is a rare condition that presents significant health risks, including stroke or myocardial infarction, and can be life-threatening if not promptly addressed. We report the case of a 42-year-old female with morbid obesity who presented with sudden shortness of breath due to a bilateral pulmonary embolism. Imaging revealed a thrombus extending from the right atrium to the left atrium through the patent foramen ovale (PFO).

Migraine, a neurological disorder often accompanied by symptoms such as visual disturbances, nausea, and photophobia, involves complex interactions between genetic and environmental factors, while vascular factors are also implicated, influenced by both genetic predisposition and environmental triggers. This case report discusses a 41-year-old male with a history of migraine with visual aura, presenting with sudden left-eye visual loss. Comprehensive ophthalmologic examination revealed a central scotoma, while multimodal imaging, including spectral-domain optical coherence tomography (SD-OCT), showed focal alterations in the outer plexiform layer.

Tamoxifen, an oral medication that blocks estrogen activity, is frequently prescribed for the treatment of advanced breast cancer and as an additional therapy following surgical removal of early stage disease. A 45-year-old female with a history of breast carcinoma treated with tamoxifen presented with sudden onset bilateral visual impairment for 4 days. On ocular examination, the patient exhibited optic disc edema with hyperemia and bilateral anterior pathway defects in visual evoked potentials.
