The management of giant omphaloceles had always been a point of interest for the pediatric surgeons. Many surgical techniques were proposed, but none of them succeeded to become the standard procedure in closing the congenital abdominal defect. We present a case of giant omphalocele in which we used staged surgical closure combined with a prosthetic patch, with negative-pressure therapy and, finally, definitive surgical closure. Even though a major complication occurred during the treatment, we were able to close the defect without any prosthetic material left in place.

Article Synopsis
  • - Complex abdominal wall hernias are tough for surgeons to fix, especially when there’s a lot of abdominal content missing, making traditional repair methods insufficient.
  • - In cases of complex ventral wall hernias, surgeons may need to first manage the disproportion of internal organs before attempting a hernia repair.
  • - A novel method involving tissue expanders was used in a successful two-stage operation for an adult with complications from a congenital omphalocele, combined with a technique called anterior component separation to enhance abdominal wall repair.

Objective: Exploration of the efficacy of treating large indirect inguinal hernias in infants and young children using umbilical double-port laparoscopy combined with extraperitoneal water injection.

Methods: A retrospective analysis was conducted on 165 cases of primary unilateral large indirect inguinal hernias in infants and young children treated at our hospital from May 2018 to May 2023. Among them, 90 cases underwent umbilical double-port laparoscopic surgery combined with extraperitoneal water injection and high ligation of the hernia sac (Double-Port Group), and another 75 cases underwent conventional three-port laparoscopic high ligation of the hernia sac (Three-Port Group).
