54 patients with severe tinnitus were treated by lidocaine iontophoresis. 25 patients suffered from a bilateral tinnitus. A total of 75 ears were treated. The ratio of male to female patients was 3:1. The average age was 45 years. All patients had got over at least one ineffective treatment. Prior to iontophoresis a complete audiologic, neuro-otologic and x-ray examination of the temporal bones was performed. In all patients the frequency and intensity of tinnitus were determined before and immediately after a treatment. In 44 patients there was no difference in their tinnitus after the treatment. In one patient the tinnitus has become lower by the iontophoresis. The value of this method in tinnitus therapy is discussed.

Although spinal cord stimulator (SCS) therapy is generally used safely to treat chronic neuropathic pain conditions, this document highlights the less reported complication of unusual neurological problems including headaches. These developed temporally after the initiation of SCS therapy despite initial positive response to pain. The mechanisms might include activation of trigeminal receptors and neuroplasticity after SCS.

Cisplatin is an established component of treatment protocols for various solid malignancies but carries a significant potential for serious adverse effects. Ototoxicity from cisplatin treatment is an important dose-limiting toxicity that manifests as bilateral, progressive, irreversible, dose-dependent sensorineural hearing loss, ear pain, tinnitus, and vestibular dysfunction. Despite the recent approval of sodium thiosulphate for the prevention of cisplatin-induced hearing loss (CIHL) in pediatric patients, structured prevention programs are not routinely implemented in most hospitals, and reducing platinum-induced ototoxicity in adults remains an important clinical problem without established treatment options.
