We report a substantial axillary lymphangioma in a fetus delivered at 38 weeks of gestation. Detailed fetal survey at 20 weeks revealed a 5.45 × 3.72 cm nonvascular cystic axillary structure without other malformations; amniocentesis was negative. Serial surveillance was performed throughout the pregnancy. A male infant weighing 3000 g with a 16 × 12 × 9 cm septated cystic mass arising from the left axilla was delivered via cesarean section. The newborn period was complicated by cellulitis overlying the mass and interval cystic hemorrhage requiring sclerotherapy and subsequent excision. Nonnuchal lymphangiomas may be etiologically distinct entities. The prognostic factors include anatomic location, presence of septa, and association with other congenital abnormalities. A thorough evaluation, multidisciplinary approach, and close surveillance should be undertaken to optimize neonatal outcomes.

Lymphangioma circumscriptum, one of the common forms of lymphangioma, can be either congenital or acquired. Various acquired conditions can lead to its causation and one of the causes might be untreated hidradenitis suppurativa. So, this report highlights the need of early interventions for hidradenitis suppurativa as lymphangioma circumscriptum can be one of its consequences later in life.

Introduction And Importance: Lymphangiomas are rare benign lymphatic malformations, typically affecting the head, neck, or axillary regions, with abdominal cystic lymphatic malformations (CLM) being particularly uncommon in adults. Abdominal CLM account for less than 5 % of all lymphangiomas and 7 % of all abdominal cystic lesions in adults with a prevalence of approximately 1 in 250,000. These lesions are commonly found by accident during imaging investigations and are asymptomatic.

It can occur in any bone and cause pathologic fractures with poor bone healing. Complications such as effusions and lymphangiomas can also develop.
