Hematomas and septal abscesses are an emergency in rhinology and require early surgical treatment due to the risk of infectious, functional, and aesthetic complications. They generally occur as a consequence of nasal trauma, although they have also been described in relation to other triggers. Accumulation of blood or pus between the mucoperichondrium and septal cartilage, causes avascular necrosis injury to the cartilage with cartilage destruction. Clinically it presents as nasal ventilatory insufficiency and facial pain. In a lower percentage, the reason for consultation was nasal dorsal deformity, epistaxis, purulent rhinorrhea and fever. We present two clinical cases with septal hematoma. Both required surgery.

Platypnea-orthodeoxia syndrome (POS) is a rare clinical condition characterized by dyspnea and hypoxemia during orthostatism, with relief in the supine position. The diagnosis of POS requires a high clinical suspicion, and its etiology stems from the admixture of venous blood, poor in oxygen, with arterial blood via a shunt. A patent foramen ovale (PFO) is the most commonly encountered anomaly at the root of POS.

Objectives: To evaluate the efficacy and safety of purse-string sutures (PSS) compared with manual compression for access hemostasis in children with atrial septal defects (ASDs) after large-caliber venous delivery sheaths removal.
Methods: We conducted a retrospective clinical data review of 271 children with ASDs who underwent transcatheter device closure through large-caliber venous delivery sheaths (≥ 8 Fr) at our institution from January 2018 to January 2023. The PSS group (n = 144) was compared to the control group (n = 127), which underwent manual compression for femoral venous hemostasis after sheath removal, focusing on hemostatic time, limb braking time, bed rest time, hospital stay, and vascular access complications.
An atrial septal defect (ASD) is a common congenital heart anomaly that results in irregular blood flow between the systemic and pulmonary circulations due to an opening in the atrial septum. Standard treatment for ACS typically involves dual antiplatelet therapy (DAPT) to prevent recurrent thrombotic events. However, in patients with recent gastrointestinal hemorrhage or significant anemia, these therapies may substantially increase the risk of life-threatening bleeding, complicating the decision-making process and often leading to conservative management strategies.
