Neuroform Atlas (NFA; Stryker Neurovascular, Fremont, CA, USA) is a useful and safe device for the treatment of broad-necked and unruptured cerebral aneurysms. Rarely does a proximal shift of both the stent and delivery catheter occur during deployment, and it can be complex to treat. We present two cases in which an NFA that had migrated proximally during deployment was successfully retrieved.

Heart failure is a clinical syndrome characterized by the inability of the heart to meet the circulatory demands of the body without requiring an increase in intracardiac pressures at rest or with exertion. Hemodynamic parameters can be measured via right heart catheterization, which has an integral role in the full spectrum of heart failure: from ambulatory patients to those in cardiogenic shock, as well as patients being considered for left ventricular device therapy and heart transplantation. Hemodynamic data are critical for prompt recognition of clinical deterioration, assessment of prognosis, and guidance of treatment decisions.

Audience: This simulation case was created for emergency medicine (EM) residents at all levels of training.
Background: Cardiac electrical storm (ES) is commonly defined as three or more episodes of sustained ventricular tachycardia, ventricular fibrillation, or three shocks from an implantable defibrillator within a 24 hour period.1 This can occur in up to 30-40% of patients with implantable defibrillators; however, it may also present in a wide variety of patients, including those with structural heart disease, myocardial infarction, electrolyte disturbances, and channelopathies.
