Integrated care is a continuum of services delivered by a system organized around the health needs of people rather than diseases. People with chronic musculoskeletal pain often live with coexisting chronic health conditions. Current care for musculoskeletal pain remains narrowly focused on individual painful conditions, despite the complex health needs of patients. We explore the challenges of delivering integrated care to people with musculoskeletal pain and coexisting chronic health conditions. We discuss these challenges in relation to 3 areas: (1) the relationships between musculoskeletal conditions and other chronic health conditions, (2) the factors that impact the clinician's capacity to provide integrated care for musculoskeletal conditions, and (3) system-level constraints that impact both the clinician's delivery of care and the patient's health care experience. We suggest ways by which clinicians and researchers may move toward better integrated care for musculoskeletal pain and coexisting chronic health conditions. .

A model of care redesign within rheumatology: A mixed methods approach integrating nurse practitioners and physician assistants.

J Am Assoc Nurse Pract

January 2025

Division of Cardiology, Department of Medicine, Duke Health Integrated Practice, Duke University Health System, Durham, North Carolina.

Background: Increasing patient demand and clinician burnout in rheumatology practices have highlighted the need for more efficient models of care (MOC). Interprofessional collaboration is essential for improving patient outcomes and clinician satisfaction.

Local Problem: Our current MOC lacks standardization and formal integration of Nurse Practitioners (NPs) and Physician Assistants (PAs), resulting in reduced clinician satisfaction and limited patient access.

Genetic diversity and selection signatures in sheep breeds.

Departamento de Ciências Exatas, Universidade Estadual Paulista (UNESP), Faculdade de Ciências Agrárias e Veterinárias, Jaboticabal, Brazil.

Natural and artificial selection in domesticated animals can cause specific changes in genomic regions known as selection signatures. Our study used the integrated haplotype score (iHS) and Tajima's D tests within non-overlapping windows of 100 kb to identify selection signatures, in addition to genetic diversity and linkage disequilibrium estimates in 9498 sheep from breeds in Ireland (Belclare, Charollais, Suffolk, Texel, and Vendeen). The mean observed and expected heterozygosity for all the sheep breeds were 0.

Neuropathic pain (NP) imposes a significant burden on individuals, manifesting as nociceptive anaphylaxis, hypersensitivity, and spontaneous pain. Previous studies have shown that traumatic stress in the nervous system can lead to excessive production of hydrogen sulfide (HS) in the gut. As a toxic gas, it can damage the nervous system through the gut-brain axis.

Few Patients Are Treated for Both Obesity and Depression.

South Med J

February 2025

From the Center for Value-Based Care Research, Cleveland Clinic.

Objectives: Understanding the epidemiology of treatment for patients with co-occurring depression and obesity can inform care quality. The objective of the study was to identify how patients with obesity and newly diagnosed depression are treated and whether treatment is associated with body mass index change.

Methods: This cohort study included adults with obesity and newly diagnosed depression who had ≥2 primary care visits between 2015 and 2020 at a large integrated health system.
