The effects of neuromuscular blocking agents on the clinical performance of supraglottic airway devices and surgical condition in elderly patients undergoing hand surgery have not been established. We evaluated the effects of rocuronium on the clinical performance of an i-gel supraglottic device and surgical condition in elderly patients undergoing orthopedic hand surgery. Patients aged 65-85 years were randomized to receive either rocuronium (rocuronium group) or saline (control group). We compared the rates of successful insertion of the i-gel on the first attempt as a primary outcome and also assessed the adequacy of i-gel maintenance during controlled ventilation, anesthetic requirement, surgical condition, and recovery time. The rates of successful insertion of the i-gel on a first attempt were 93.1% in the rocuronium group versus 82.1% in the control group ( = 0.423). Peak inspiratory pressure (PIP) was lower in the rocuronium group than in the control group (15.2 vs. 17.9 cmHO, respectively, = 0.028). Spontaneous breathing was less common in the rocuronium group (24.1% vs. 57.1%, respectively, = 0.011). The requirement of additional fentanyl to suppress spontaneous breathing or patient movement was less in the rocuronium group than in the control group (24.1% vs. 50.0%, respectively, = 0.043). Surgical condition did not differ between the two groups. Recovery time was shorter in the rocuronium group than in the control group (8.4 vs. 9.9 min, respectively, = 0.030). Rocuronium did not enhance the success rate of inserting the i-gel or the surgical condition in elderly patients. However, using rocuronium reduced PIP, the frequency of spontaneous breathing, the requirement for additional fentanyl and patients' recovery time.

Osteopetrosis, also known as marble bone disease, is a genetic condition characterised by generalised sclerosis of bones. The osteoclastic cells responsible for bone resorption are defective and nonfunctional. These patients tend to have low bone quality in spite of increased bone deposition and thus experience multiple fractures during their life span.

Chronic instability at the lumbosacral junction, particularly between the L7 vertebra and the sacral bone, presents significant challenges in veterinary orthopedics, especially for large breed dogs. This condition frequently results in severe pain, neurological deficits, and mobility impairments, prompting the development of various surgical techniques aimed at effectively stabilizing the affected area. A critical evaluation of the literature on surgical stabilization of the lumbosacral spine in dogs reveals the clinical applications, outcomes, and future directions in veterinary spinal surgery.

Department of Neurobiology, Care Sciences and Society, Center for Alzheimer Research, Division of Clinical Geriatrics, Karolinska Institutet, Stockholm, Sweden.

Background: [F]FDG PET is essential since it allows us to differentiate between different dementia disorders/types, revealing distinct neurodegenerative patterns in those predisposed to the condition. Individuals with Autosomal Dominant Alzheimer's Disease (ADAD) have a predictable age of onset, enabling the study of cognitive and pathological changes before clinical manifestation. Our objective was to investigate temporal course and regional links between cognition and glucose metabolism as a measure of early synaptic impairment in ADAD.

Introduction: Lymphedema, a debilitating characterized by localized fluid retention and tissue swelling, results from abnormalities in the lymphatic system. In the case of primary lymphedema, this condition is attributed to malformations in lymphatic vessels or nodes, and it is marked by a relentless progression leading to irreversible tissue fibrosis after repetitive inflammation. Many questions regarding its treatment, such as the choice of the type of intervention and the timing, still remain unanswered.

Alzheimers Dement

December 2024

Florida State University, Tallahassee, FL, USA.

Background: Addressing femoral neck fractures resulting from ground-level falls in older adults with Alzheimer's disease (AD) involves a personalized treatment plan, leading to a substantial economic burden on the healthcare system. The debate surrounding the advantages and disadvantages of surgical interventions versus non-operative approaches for femoral neck fractures in older individuals with AD remains a topic of active discussion.

Method: In this retrospective cohort study, the total medical expenses associated with operative and non-operative therapies were compared while adjusting for patients' demographics and baseline health conditions.
