Background: Isolated complex perianal fistulas, without luminal evidence of inflammatory bowel disease in the gastrointestinal tract, pose diagnostic and treatment dilemmas for gastroenterologists and colorectal surgeons. For patients who develop recurrent complex fistulas, a presumptive diagnosis of Crohn's disease may be made. It is unclear whether these cases of isolated perianal disease in the absence of luminal inflammation truly represent isolated severe cryptoglandular fistulas or rather an early presentation of Crohn's disease.

Purpose: Patients with Crohn's disease (CD) frequently undergo multiple abdominal operations, which increase the risk of incisional hernia repair (IHR) and associated mesh-related complications. Patients with intra-abdominal fistulizing CD (FCD) may be more susceptible to mesh-related complications than patients with non-fistulizing CD (non-FCD). The primary objective was to evaluate the risk of reoperation due to mesh-related complications after IHR in patients with FCD and non-FCD.

Four of them had had inflammatory lesions in armpits and groins.

Article Synopsis
  • The study highlights the rise of carbapenem-resistant Enterobacter chuandaensis, a rare member of the Enterobacter cloacae complex, complicating infection treatment.
  • Researchers identified a specific strain, FAHZZU5885, from a patient in ICU, which carried multiple resistance genes and exhibited resistance to most antibiotics except a few.
  • Genetic analysis revealed that the strain harbored two different plasmids containing carbapenemase genes, raising concerns about its potential to spread and contribute to further resistance issues.
