Introduction: Onychomatricoma is a rare subungual tumor composed of stroma and nail matrix-like epithelium. Only seven cases of the pigmented variant have been reported in the literature.

Case Report: We present a 62-year-old male patient with a 2-years history of a single fingernail thickening, deformation, and hyperpigmentation. He had been treated for onychomycosis, without improvement. Due to the increasing hyperpigmentation, and to rule out tumoral entities, including melanoma, a color Doppler ultrasound examination of the nail was requested, which showed a hypoechoic tumor of the nail matrix and bed, with hyperechogenic linear images inside, suggesting the diagnosis of onychomatricoma. Onychectomy and partial matricectomy were performed. Histological evaluation confirmed the diagnosis of onychomatricoma. Discusion and conclusion: Histological evaluation is the gold standard for the diagnosis of onychomatricoma. However, ultrasonography represents an easily accessible, non-invasive diagnostic tool for the initial evaluation of these tumors, especially in unsuspected atypical clinical presentations.

Article Synopsis
  • Scientists came up with the terms "onychofibroblast" (nail-specific cells) and "onychodermis" (nail-specific skin) to describe special parts of the nail that are different from regular skin.*
  • Recent research shows that these nail cells and skin areas are important for how nails form and can affect nail health when they have problems.*
  • They found that certain genes in the onychofibroblasts help with nail growth and that tumors related to nails come from these special cells.*

Article Synopsis
  • Onychomatricoma (OM) is a rare benign nail matrix tumor that requires surgical treatment, but there’s limited data on outcomes.
  • A study reviewed 62 patients over 25 years, focusing on the relationship between the tumor's location, size, and postoperative results.
  • The findings revealed that tangential excisions led to normal nail regrowth in 71% of cases, but tumors thicker than 3 mm or covering the entire nail plate had a higher risk of complications.

Article Synopsis
  • Dermoscopy of the nail unit, or onychoscopy, is a non-invasive technique that enhances clinical diagnosis of nail-related conditions, currently used for both inflammatory and cancerous issues.
  • Intra-operative onychoscopy allows doctors to visually inspect nail structures during surgery after the nail plate is removed, providing more detailed insights.
  • A systematic review examining 19 studies with 218 cases highlighted conditions like melanoma and squamous cell carcinoma, but noted a limited number of studies with low evidence, emphasizing that it should complement, not replace, traditional histologic examinations.
