Wrist arthrodesis has been used successfully for the management of severe wrist flexion deformity when soft tissue procedures would not provide adequate correction. However, in athetoid type cerebral palsy which has a component of involuntary movement, the outcome of wrist arthrodesis has not been discussed much. We present our experience in 2 athetoid type cerebral palsy patients who underwent wrist arthrodesis due to severe involuntary movement of the wrist. One patient had a nonunion and both patients had unexpected aggravation of involuntary movement in the adjacent joints. Secure fixation using a pre-contoured plate is necessary and preparation for iliac bone grafting should be considered as proximal row carpectomy is usually not necessary in these patients. In addition, although single-event, multi-level surgery is advocated for patients with cerebral palsy, potential additional procedures for the adjacent joints should be discussed preoperatively because unexpected aggravation of involuntary movement of adjacent joints can occur after stabilization of the wrist.

Background: Kienböck's disease (KD) is a progressive condition characterized by avascular necrosis of the lunate, leading to carpal collapse and wrist dysfunction. Scaphocapitate fusion (SCF) is a salvage procedure often used in advanced stages of KD to stabilize the wrist and alleviate symptoms. This study aimed to evaluate the clinical and radiological outcomes of SCF in stage III KD, with a specific focus on the impact of lunarectomy on postoperative pain and function.

Method: Between September 2013 and April 2024, 106 consecutive patients were screened, with 64 consenting to participate. Final analysis included 56 patients (32 stage IIIB and 24 stage IIIC) who underwent scaphocapitate arthrodesis with lunate excision, utilizing distal radius bone grafting stabilized by Herbert compression screws.

Pediatric patients can pose challenges with regard to both diagnosis and obtaining an accurate history and physical examination, as well as in treatment strategies, as options become limited when physes remain open. This case report examines a 12-year-old boy who sustained a radial neck fracture after a ground-level fall. Although his elbow injury was treated appropriately, he developed wrist pain that progressed to static volar intercalated segmental instability deformity in the context of a carpal instability nondissociative wrist.

Surgical reconstruction offers the potential to restore motion in the hand, wrist, and elbow for those with deficits following cervical spinal cord injury. Techniques such as tendon transfer, tenodesis, and arthrodesis-often used in combination-are well-established strategies for enhancing upper extremity function.

We also report the successful clinical use of a vascularized proximal radius bone graft (VPRBG), supplied by the RA, in a complex case involving a massive osseous elbow defect.
