Eumycetoma, a chronic fungal infection caused by , is a neglected tropical disease characterized by tumor-like growths that can lead to permanent disability and deformities if untreated. Predominantly affecting regions in Africa, South America, and Asia, it imposes significant physical, social, and economic burdens. Current treatments, including antifungal drugs like itraconazole, often show variable efficacy, with severe cases necessitating surgical intervention or amputation.

Article Synopsis
  • Mycetoma is a type of infection caused by fungi or bacteria, and identifying the specific agent is crucial for treatment success.
  • This study analyzed 32 mycetoma samples from patients, using advanced genetic techniques to identify the pathogens, focusing on different regions of DNA for various grain types.
  • The findings revealed a 59.4% identification success rate, with the fungal pathogen Madurella mycetomatis being the most common, and two potential new agents were also discovered.

Article Synopsis
  • * Current treatments for eumycetoma, largely relying on antifungal drugs and surgery, have low success rates and high chances of relapse.
  • * Researchers have synthesized 47 new chemical compounds that show better effectiveness and lower toxicity than existing treatments, particularly one compound that performed similarly to the standard drug itraconazole, suggesting they could be safer options for treating eumycetoma.

Background: Eumycetoma is an implantation mycosis characterised by a large subcutaneous mass in the extremities commonly caused by the fungus Madurella mycetomatis. Despite the long duration of treatment, commonly a minimum of 12 months, treatment failure is frequent and can lead to amputation. We aimed to compare the efficacy of two doses of fosravuconazole, a synthetic antifungal designed for use in onychomycosis and repurposed for mycetoma, with standard-of-care itraconazole, both in combination with surgery.

The World Health Organization, in response to the growing burden of fungal disease, established a process to develop a fungal priority pathogens list. This systematic review aimed to evaluate the epidemiology and impact of eumycetoma. PubMed and Web of Science were searched to identify studies published between 1 January 2011 and 19 February 2021.
