Published: June 2021

Objectives: To describe and assess a novel technique of complete endoscopic combined transcervical-transoral fistulectomy (ECCOF) in the management of pediatric complete second branchial cleft (BC) fistula tracts (SBCFTs).

Methods: A prospective single-center consecutive case series of SBCFTs was designed. Course and angles of inclination of the tract towards the pharyngeal wall were assessed using CT fistulography. Complete endoscopic fistulectomy was performed using three levels of dissection via ECCOF. Technique, advantages, complications and recurrences were assessed.

Results: Five children with a mean age of 4.1 ± 0.96 years and seven SBCFTs were included. Four were left-sided fistulae (57.2%), while three were right-sided fistulae (42.8%). The average angle of deep inclination of the fistula tracts between the first and second parts of the fistula tracts (at the carotid bifurcation) was 143.57 ± 10.92°. Complete visualization with safe dissection in all three levels of ECCOF was obtained for all fistulae. No recurrence or complications were observed with an average follow-up of 35.85 ± 22.13 months.

Conclusion: Endoscopic management of SBCFTs appears to be effective and safe. It avoids the prerequisite for wide or double incisions and enables an excellent view of the surrounding structures, which leads to fewer complications and recurrences.

Aortoenteric fistula (AEF) is an abnormal connection between the aorta and the adjacent gastrointestinal (GI) tract and is often misdiagnosed in clinical practice. We present the case of a 65-year-old male, who presented with upper GI bleeding and melena. The patient underwent upper and lower GI examinations with no conclusive findings.

To evaluate the feasibility of liver tract embolization after transhepatic biliary drainage using a biodegradable polymer plug (IMPEDE-FX, Shape Memorial Medical, Santa Clara, CA, USA).In a retrospective observational study, 15 plug embolizations were performed in 13 patients at risk for tract-related adverse events (AEs). Risk factors included coagulopathy, cirrhosis, central bile duct puncture, previous drain-related bleeding, malignant obstruction, large tract diameter, or multilevel strictures.

Purpose: This study aimed to evaluate the long-term efficacy of the ligation of the intersphincteric fistula tract (LIFT) procedure in treating high transsphincteric fistulas.

Methods: We conducted a retrospective study to evaluate the success rate of LIFT treatment in 82 patients with high transsphincteric fistulas involving at least 1/3 of the external sphincter. This study was carried out across 2 centers from November 2009 to February 2023.

Background: The treatment of transplant ureteral stricture (TUS) has been a great challenge, and there is limited experience with indocyanine green (ICG) fluorescence-guided robotic Boari flap-pelvis anastomosis to identify ureteral stenosis segments (especially long-segment) and their postoperative blood supply. We report case series of ureteral strictures treated with ICG fluorescence-guided robotic Boari flap-pelvis anastomosis in our center.

Case Description: We retrospectively collected clinical data of six patients diagnosed with long-segment even full-length TUS who underwent robotic Boari flap-pelvis anastomosis with the assistance of modified distribution of robotic ports and ICG fluorescence between June 2022 and June 2024, focusing on postoperative renal function, stenosis recurrence, and urinary fistulae.

This case report describes Crohn's disease complicated by squamous cell carcinoma in an enterocutaneous fistula. A 48-year-old male patient was diagnosed with Crohn's disease 24 years ago and has undergone five surgical operations. An enterocutaneous fistula originated from the midline abdominal wound 11 years after the onset.
